Suggest Treatment For Osteoporosis While On Reclast, Prolia And Forteo

Question: 69 yr old female with osteoporosis, broken foot w/o accident or injury. Less than one year pelvis broken in 3 places, no accident or injury. Have been on Reclast, Prolia, and 6 months of daily Forteo. My rheumatologist has no other way to help me. Seeing oncologist, 12 yr. breast cancer survivor with 5 yrs of oral chemo, for help. There are doctors in the area affiliated with Mayo Clinic. I need a more aggressive treatment plan. what would you advise for treatment? One year ago had bone scan and bone biopsy to rule out bone cancer.

recent bone density -2.5. Two years ago -2.9

Well, You are already on the best treatment for osteoporosis at the moment. Prolia and Forteo are usually the last resort for patients who have had osteoporotic fractures. Also supplement it with calcium and Vitamin D supplements

For now, what we can do is for you to continue with Forteo for at least 6 more months and then we can overlap it with Zolindronic acid. Forteo usually helps the bone forming cells and Zolindronic will stop the bone breaking cells. So that combination would work better....

Thank you, Dr., for your prompt response. A follow up question, is there a cancer drug that can be used for ongoing broken bones? This is a quality of life issue for me. I am retired and wanting to travel. In Oct. I broke my pelvis in 3 places that took 4 months to heal and now the foot, also while I was on vacation. I am seeing my oncologist in a few days and would like your opinion on cancer meds for severe osteoporosis, maybe something for bone cancer.
Also, since both breaks were when I was on vacation, should I not be lifting the suitcase off of airport baggage claim area. Also, should I not be lifting heavy groceries? I am recently living alone and lifting more often. My regular exercise program is therapeutic yoga 75 minutes 1x week and water aerobics 60 mins. 1x a week. I thought this would help me not to break the bones. Thank you

Well,firstly you don't have bone cancer so we cant give you any chemotherapeutic drug for it.It.Osteoporisis is a different condition which may have been worsened by your cancer and is causing easy breakage of bones as it dissolves the bone matrix inside.

2. you should refrain from lifting heavy objects especially after fracture.Groceries are ok but avoid lifting suitcases

3. As for the exercises they are your main bit especially.for the bones which have received fracture. I would suggest you go.for physiotherapy sessions and focus more on the areas which have received fracture to strengthen the muscles around it. Muscle strengthening at spine,ankles,hips will help you in preventing further falls and breakage

For the medicine part you are already on the best treatment so far..Zolindronic acid we can use a bit later down the line
