Question: Still can't sleep on 50mg of temazepam I take 450mg of efexor daily. Still get negative thoughts & stress out easily. Get frustrated & need to escape situations. Can't stop angry thoughts. Can I take higher dose of temazepam ?

Hello thanks for asking from HCM

I can understand your concern. You are on Temazepam 50 mg per day, 450 mg venlafaxine per day. You are not able to stop angry thoughts, you need to escape situations. You are not able to sleep properly even with 50 mg dosing.

As per my opinion the doses of Temazepam you are taking already high and still you are not able to sleep properly. Most likely you have developed tolerance to this drug and as per my opinion the dose should not be increased. Increasing the dose may help in sleeping but ultimately high doses will fail to induce sleep.

I would advise you to consult your psychiatrist for the problem of sleep and poor control of depressive symptoms. Some other antidepressants like MIRTAZAPINE can help to induce good sleep and it will help in controlling the depressive symptoms also.
Trazodon is one more alternative drug. The drug helps in good sleep in individuals who fail to respond to conventional hypnotics. The drug is also a non-abuse potential drug so safe to use. Most likely the reason of poor sleep is depressive symptoms and control of depression will result in improvement of sleep.

Alternatively for immediate relief medicines like zolpidem, zopiclone can be used along with your drugs. These drugs help to induce sleep and temazapam will maintain it.

I would advise you to consult your psychiatrist regarding these drugs. These drugs are prescription medicines and need proper prescription.

Apart from that try to improve your sleep hygiene as it will help in improving sleep.

- Fix your time to go to bed.
- Use your bed only for sleeping and not for watching TV or reading anything.
- Avoid tea, coffee, any activating substance or even alcohol in evening hours.
- Bathing with hot water in evening will help in getting good sleep.
- Don't think about not getting sleep again and again. Excessive worries about not getting sleep or ruminations result in further deterioration. If you don't get sleep with in 10 minutes of going to bed then get up from bed.
- Light exercise in evening will help in getting you good sleep.
