Brief Answer:
As the opening of the spine is already narrowed, surgery is a must...

Detailed Answer:
Hi,

I understand your concerns.

At your age (79), it is often difficult to make decisions regarding best treatment strategy especially when it comes to spondyloarthritis (arthritis of the spine).

You are saying that arthritis has caused narrowing of the spine and difficulty walking. And I can see that your case is very delicate especially when taking into account your age.

Pain medications (taken orally or applying locally) cannot help alone with your current arthritis problems. Physiotherapy neither. That's why your doctor has advised surgery. The aim of surgery is to open the narrowed spine, thus, the pressure which has been put onto the nerve so far will be gone and hopefully, you won't have difficulty walking.

Let me know if you have further queries!
Good luck!
