Question: muscle cramps and stiffness both day and night. cramps start in the thigh and hamstring and travel down to the calf and foot.

First of all, please note that there are numerous possibilities which can cause such muscle stiffness and cramps, some commonly observed ones in the day to day practice are anemia secondary to iron deficiency, Vit-D3 and Vit-B12 deficiency, Electrolyte imbalance (mainly Calcium, Magnesium and Potassium) and diabetes.

If they're already ruled out, then other possible causes should be look in to, namely disturbances the blood circulation of the lower legs (a Doppler ultrasound scan should be done look into it in details).

As I see, you've already tried Theraworx. I'd suggest getting these things checked out first so that the exact cause can be narrowed down, specific treatment can also be initiated based on the findings. Please discuss this with your doctor and proceed further.

Well, then the blood circulation assessment (by the Doppler scan of the veno-arterial system) of the lower limbs should also be considered as a part of primary evaluation (along with those other investigations as explained earlier).

Since there's also a preexisting cardiac condition and poor circulation with SOB, I'd recommend a fresh 2D-Echocardiogram along with a Stress test to determine the present cardiac functionality. If the cardiac pumping and functionality are markedly reduced, that can even compromise the overall blood supply to other parts of the body, hence give rise to such symptoms.
