Suggest Treatment For Mild Endometriosis And Infertility Problem

Question: Hello, I'm 30 years old and married for 5 years. I have regular periods(26-28 days) and normal ovulation documented through follicular scans. Initial scans showed no
cysts, and a healthy reproductive system, with all blood-work(FSH/LH/Prolactin/TSH/AMH=3/Blood Sugar) as normal. HSG was also done and confirmed patent fallopian tubes. My husband's Semen analysis came back perfect.
I was hence diagnosed with "unexplained infertility".
I was then prescribed Clomifene Citrate 100mgx5days as part of my 1st IUI plan. I developed OHSS and had to wait out for 3-4 months for all the cysts to go away.
In the meantime, I voluntarily got my serum progesterone checked in 2 consecutive cycles 7 days past LH surge, with results : 7.2 ng/ml and 6.7 ng/ml respectively.

Following this I was prescribed Duphaston 10 mg twice a day for 3 months. Inspite of this, my CD 21 s.progesterone showed nothing beyond 7 ng/ml. But this didnt seem to concern my doctor much.
Later, when I came back for the scans, I had a 2.8cmx2.3cm endometriotic/heamorrhagic cyst in my left ovary. So the diagnosis now was "mild endometriosis otherwise unexplained".

I just had a failed 2nd IUI. My periods started at 14 days past IUI, even when my course of Naturogest VT 200 was not over. On day 2 after the failed cycle, I went for a scan and the endometriotic cyst was now down to 1.7cmx1.3cm.

The following are my questions:

1. Can I actually have my period start even while on progesterone? Does that mean no form of progesterone (oral/vaginal tablets) is absorbed by me and I'm still
deficient?
2. Is it impossible to conceive with CD 21 progesterone being just 7ng/ml?
3. When no cyst was seen before taking clomifene, how can I be sure that the existing cyst is not a remnant of the OHSS?
4. I do not have painful or heavy flow periods to say that I may have endometriosis. Can endometriosis just be diagnosed by detecting an endometriotic cyst seen only after OHSS? If it were an endometriotic cyst, it wouldn't have reduced in size. Am I correct to think this way?
5. Even in case my issue is (a) low progesterone, and (b) mild endometriosis, can IVF really bypass these issues?

I read your query and I understand your concerns.

Following is my

1. Your OHSS cycle was in past. Those cysts wont have remained till now. Current cyst has nothing to do with your OHSS cycle

2. Mid cycle D21 progesterone levels of 7 is not a problem. You can still conceive with this levels as these are not reliable measurements.

3. This does not appear to be endometriosis as they wont become big so soon as they were not there earlier. It wouldn't have reduced so soon as you correctly guessed.

4. Let me know where you are taking these treatment. I can advice better centre if I know in your town.

Thank you for your advice.

If progesterone levels is not a limiting factor, then what could be the reason that I start my periods even when the supplements' course is not over? Does it mean that any mode of progesterone tablets (oral/vaginal) is not working out for me?
What would be the best way to go about this?

I'm taking Paxidep CR12.5 for depression. My psychiatrist says that it is the lowest dose of SSRI and safe for conception/pregnancy. But my previous gynae seemed to not favor this drug. She had my Prolactin levels checked time and again, and it seemed normal (13,14,17..). Please let me know your opinion on this. Does this SSRI in anyway hamper the fertilization/implantation?

Progesterone does not seem to be problem.

Probably the dose needs to be increased. Or better option is to take intermittent injections of progesterone.

Your paxidep does not interfere with progesterone. You can continue to take it.
