Question: I have had lower back pain for several years and have been experiencing shortness of breath, dry mouth, sleepiness during the day, The shortness of breath is almost debilitating.

Regarding your concern, I would explain that the shortness of breath does not seem to be related to the lower back pain.

It could be related to heart failure or a lung disorder.

The increased sleepiness could be related to different possible causes (heart failure, low oxygen levels, pulmonary disease, metabolic disorders, sleep apnea, etc.)

For this reason, I would recommend consulting with your attending physician for a careful physical exam and some tests:

- a resting ECG and a cardiac ultrasound to examine your heart function and structure
- a chest X ray study and pulmonary function tests
- complete blood count, PCR, ESR for inflammation
- a lumbar spine X ray study to examine your lower spinal column
- kidney and liver function tests
- fasting glucose.

You should discuss with your doctor on the above issues.
