Suggest Treatment For Low Platelet Count With Few Giant Platelets

Question: Hello,

I have had low platetets (ITP) since 1997, the lowest being in the 100's. Recently I went for my yearly checkup and blood work came back with low platelets again, and also a note of rare giant platelets seen. I have an appointment in a week to see an oncologist. The waiting is killing me and I of course am doing all kinds of research about giant platelets online, and some of the causes are alarming.

Some history:
chronic ITP since 1997 - idiopathic
hospitalization for Mono in 1990 - enlarged spleen but did not rupture
2 C-sections - 2nd in 1998 reopened and healed inside out due to infection
Septic Pelvic Thrombophlebitis in 1998 after birth of daughter - severe blood clots with PE in hospital for two weeks, extremely high fevers - Vancomycin, Heparin treatments
August 2013 - 13 platinum coils in gonadal and renal veins for CPS
February 2014 - 12 platinum coils in iliac and surrounding veins for CPS
Various cysts drained and removed, one behind right ear, one on right foot, etc., all benign
Very dense breasts, multiple fluid filled cysts, all benign, several removed and drained
Recent swelling of thyroid on an off from past three months, recent ultrasound revealed thyroid cysts, chest xray clear, thyroid blood tests normal.
Extreme tiredness, no energy, some bruising, but not major, some night fevers, feeling of something is not right...

My concerns are lymphoma or myelodysplastic syndrome, etc. I just would like to get a second and QUICK opinion based on your expertise. I know my diagnosis will depend on a lot of other tests and actually seeing my oncologist.

What are your thoughts? Any other ideas of what this might be? Your help is greatly appreciated! Thanks so much!

Thanks for your quick answer. What other reasons for giant platelets other than ITP??? I know this is not from anything hereditary.
The only notes on blood work that I can see on my reports are as follows:

Monocytes/100 Leukocytes: 9
Platelets: 119
Hematocrit: 43.8

Peripheral slide reviewed:
- RBC: normal morphology
- Leukocytes: normal morphology
- Platelets: no clumping, rare giant platelets seen, few big platelets, can be
consistent with increased peripheral destruction as seen in ITP

No need to worry about other disease.

Detailed Answer:
Hi, dear
I have gone through your question. I can understand your concern.
You have few giant platelet only. Your peripheral smear examination report is normal other than platelet.

No need to worry about lymphoma or myelodyplasplastic syndrome.

There are many causes of giant platelet. Most commonly it is seen non specifically.

Many hereditary cause like XXXXXXX soulier syndrom and gray platelet syndrome are there but they are very rare. You don't have any history of hereditary syndrome. So nothing to worry about that. Just be relaxed.
Just repeat your platelet count after a week.
Also check prothrombin time once to rule out other bleeding problems.

Your platelet count is slightly low so not a cause of big worry.

If it decreases further or you have any signs bleeding then consult your doctor urgently.
