Suggest Treatment For Itchy Rash On Genitals

Question: Hi Doctor, i have a rash on my genitals which is very itchy and surrounding areas ie(buttocks & thighs), the rash has caused my scrotum to swell to double its size which is painful at times , i also have a lump under my armpit (which i presume is a swollen gland), i have been self medicating but am now concerned , do you know what this could be ?.

Thanks for the query.
Probably you have developed a fungal infection called as tinea cruris.
Your description matches with that.
The armpit lesion can be a furuncle.
It is a bacterial infection.
Can you please upload a photograph of both the areas.
This will help me in giving a closer diagnosis and a better line of management.
Also I would like to know the treatment taken by you in past.

This is the the rash which is all over my buttocks,inner ans outer thighs , i cant really see if that is what the rash looks like on my scrotum because it is so swollon but its sure itches the same as everywhere else.While i was away on holiday where this all started +- 6 days ago , i went to the doctor there where she said it was caused from wearing a wet costume and its basically the same as nappy rash (obviously not) she then have me a cortisone injection which she said would ease the itching , which it did not , the rash has got extermely worse , i have been using a cortisone cream and another cream which has zinc oxide in it (not to sure of the name) nothing seems to be helping.can you help ?

Thanks for the follow up.
Cortisone will not help you.
It is a steroid and will aggravate the condition.
You need treatment with antifungal cream.
I usually treat such patient with clotrimazole cream, to be applied twice daily for 15 days.
This gives excellent results.
