default
Posted on Mon, 27 Oct 2014
Question: Hi there, previously i wrote in with the concern about the itch around my anus. I had seen a dermatologist and also a gp regarding this issue but nothing seemed to help. The dermatologist prescribed me forbancot and protopic but my condition got worse. The gp prescribed me melomet cream but it also doesn't seem to be getting any better. Recently it got worse and my skin on top of my butt crack split open. And i also realized tiny holes around the skin. I wonder what's wrong with me. The itch had been keeping me awake and this condition had been bothering me about 6 months and i really want it to be cured so i can have a good night's rest. I have attached a pic for your dignostic. I really need answers and solution badly.

Brief Answer:
Intergluteal Dermatitis.

Detailed Answer:
Hi.
Thanks for your history and good quality pictures.
This is intergluteal dermatitis.

Yes, I would like have some information before answering your query. This will assist us both::
* For how much length of time you have got this problem?
* How was it started, I mean what were the symptoms in the initial period?
*Is there any problem related to your abdomen like indigestion, colitis or IBS ( irritable bowel syndrome)?
*What is the treatment have you taken? And if yes any results?

We have a facility that you can attache a nice photographs:
In my opinion, this is a condition of dermatitis in the intergluteal region.
The exact cause of this condition is not known, but this is treatable and can be cured.
It usually starts as a itch, particularly n summer season. This is probable due to inability to clean up this area nicely with water, retention of the sweat, soap and so on.
The itching causes small injuries and mild infection. The area may weep, gives fluidly secretion and causes more itching , particularly at night.
Patients starts using whatever ointments are available or suggested by friends, family or local Doctors. Feels good, but the disease remains there to recur or have vexing and vanning.
Many patients have some or the other problems with digestion or motion.

I do advise all such patients as follows::
-Clean the area with plain water as frequently as possible.
-Apply liquid Povidone Iodine locally to the whole area and allow it to dry.
No other ointments or lotions/ soaps / chemicals are needed.
-A small course of an antibiotic and Metronidazole for about 5 days ( you need a prescription please- of a local Doctor)
-This is basically a disorder due to deficiency of vitamins and minerals that the integrity of the skin is lost. So you have to have therapeutic dosage of Vitamin A and D and to continue as multivitamins which are available as anti-oxidant medicines for about a month or so.
_ we have seen the best results in almost all patients with this therapy- thanks again for the photograph for correlation.

Take care, get well soon and permanently.

I had this problem for almost 6 months, it just started out itching all of a sudden one day. The skin was still fine then, then it lichenfied and then it toned down and after, it flares up all of a sudden with a tiny bump, in certain areas which the bumps itches terribly. The little "holes" or "pores" begin to surface, i tries squeezing thinking it might be blackheads or whiteheads but nothing comes out. I have no ibs nor indigestion, but could it be linked to my renal failure condition, like maybe some of meds i am taking like diuretics or even antihistamine? All treatments from gps and specialist have no effect so far except from getting from bad to worse. Nowadays, slight sweat or even feeling of moving bowels will send the area in an intense itch. It's not only affecting the top of mu butt crack but it's also around the anus.

Brief Answer:
Follow the advise

Detailed Answer:
Hi.
Thanks for your feedback.
This may be related to renal failure conditions, not with any medicines.

Once a particular area goes into such conditions it becomes a routine problem as the itching starts off the condition..

Please follow the way I have advised, this will get OK.
You Should tale the medicines in consultation of the Nephrologist about its dosage and duration.

Brief Answer:
The pores are normal but pronounced

Detailed Answer:
The wound is in the area of friction- continuous one due to its inter-gluteal position.
The pores are pronounced normal ones, enlarged due to itching, disease and loss of integrity.
Do not touch, itch or squeeze. The more you fiddle , the more will be pronounced problems.
Seen the additional picture you have posted.
Follow the treatment plan and you will be fine.
