Question: What do you consider to be the best treatment for IC with Hunners ulcers? And what is on the horizon that looks most promising, and when do you estimate that it will be available?

Several options available.

Hello XXXXXXX and welcome to HCM.
As an Urologist, i can fully understand your concern.
IC is not easy to treat and experience for a person having it.
Hunners ulcers are best treated with Lasers.
If the bladder installations don't help a patient, stop it.
Pentosan polysulphonate (ELIRON), is standard treament by all Urologists.
Any treatment must be carried out, only with advise by the Urologist, who's monitoring the treatment. An alpha-blocker is always given.
Next alternate treatments available are opioids for pain relief, Botox injections into bladder muscle, Yoga and meditation for stress relief, Amitryptiline, Neurostimulation, cystoplasty and even, bladder removal has been considered.
All treatment must be in consultation with Urologist only.
