Brief Answer:
Internal hemorrhoids treatment strategy as following...

Detailed Answer:
Hi,

I am sorry for the health problems you are going through.

Now, from what you are describing, I see that there a few health problems you are facing that requires different treatment strategies:

- demodex infestation which has been confirmed and correctly treated by with a good personal hygiene, and Ivermectin. Currently, you report there are a large amount of demodex dying off and that's great.

Using oral ivermectin is advised in case of no further improvement. However, should discuss with your doctor for this prescription as should evaluate your health problems first.

- internal hemorrhoids is a different issue and has nothing to do with demodex infestation. They are more related to bad bowel movements (constipation) most probably due to a bad diet (low in fiber and water) or any medication you've been using on regular basis.

However, I need to know if the problems with internal hemorrhoids has started now or you have had them before (prior to Demodex infestation)?

The killing pain you have due to internal hemorrhoids is managed by taking spasmex or spasmodil or even buscopan for short term as they will make your bowels lazy and will aggravate constipation.

Sometimes, more potent analgesics can be used together with local (per rectum) suppositories to ease the local inflammation and pain.

Try to avoid constipation by:
- drinking more plain water
- avoid foods/drinks that would make your bowels lazy
- be more physically active
- have more fiber rich foods
- fiber supplements containing psyllium and prunes can help
- sena supplements and lactulose can be used on short term to induce and ease bowel movement
- rectal enema can be used in case of very hard stool

Hope this helps.

Hi, thank you for your response. I've actually had the hemorids off & on for many years. The acute pain over the last couple of days is due to the Demodex that are now in there causing me severe pain.

Just within the last hour I decided to do a rectal douche of lukewarm water & was horrified to expel out well over a dozen Demodex mites! I am in shock. I can still feel movement & discomfort in there however. Should I keep going? Is there a better way to rid my rectum of these disgusting mites??

What you are doing is the best unless...

Detailed Answer:
Hi back my dear,

I am sorry about the bad situation you are in.
That's the best thing you should try and you are doing well.

However, you should not be in pain, that's why I recommended more potent analgesics that can be prescribed by your doctor.

If the pain is unbearable, you should go to ER for immediate medical assistance.

Wish fast recovery.
