Suggest Treatment For Intermittent Recurrence Of Fever

Question: Son has fever (<100) from last 3 days (today is 4th day). Given Megadol 2 times on first day of fever at evening and mid night only.
Want to understand weather it is serious or we shall wait for few more days. Also his vaccination is due for Typhoid 1, what to do?

Weight is 13 kg and age is 2 years. Fever is not continuous. It comes for appx 3-4 times in 24 hours.

Hi... I have just answered your query in which weight and age were not mentioned, but here in this answer I would again reiterate the fact that you need not worry unless the kid is having the danger signs which I have explained earlier.

Viral illness will be a continuous fever in first 1-3 days and then gradually the fever will space out in another 2-3 days and by 7-9 days it will become normal. Usually rather than fever, what is more important is the activity of the child, in between 2 fever episodes on the same day. If the kid is active and playing around when there is no fever do not worry about duration of fever.

Suggestions on medications -

1. Paracetamol can be given in the dose of 15mg/kg/dose (Maximum ceiling dose 500mg per dose) every 4-6th hourly that too only if fever is more than 100F. If you can get back with the strength of Paracetamol (Megadol) you are having with you I will suggest you the appropriate dose.

2. I do not suggest using combination medicines for fever, especially with Paracetamol.

3. Many a time - doctors prescribe Mefenamic acid for fever control - Please do not use it. Mefenamic acid is an NSAID. It carries the risk of gastritis and renal problems in the long run. Please do not use Mefenamic acid for fever control.

Thanks doctor XXXXXXX for prompt, detailed response and reference. Patient's age is 2 years 0 Month and weight is 13.2 kg.
Fever was around 101F for first day when we used Maftel (Mefenamic acid). We will avoid it for fever control in future. Wish there is no major risk for our earlier usage.
Please suggest Megadol (250mg/5ml) dose.
