Suggest Treatment For Intermittent Migraines

Question: I would really like to speak with a neurologist please due to my current condition I thought I was having migraines 5 years ago I had a ct scan that showed a white lesion last month I had a mri done that showed at least 20 more my dr do sent think it's ms or migraines and . is testing me for cadalis I'm very worried I've been experiencing new things

I read your question carefully and I am sorry about what you are going through.

Judging from the limited information I have until now, I would say that the most probable alternative would be multiple sclerosis.

Cadasil can present with migraines and white matter lesions but usually there is a family history for it and also usually the lesions are not so many so early, but at a later age then 20. Migraine itself can manifest some changes as well, but again not so early, usually later on.

Of course if you could provide some more details on your symptoms and perhaps full MRI report I might be more helpful.

my mother has this problem also as well as her sister who has ms. as for me I've been having this problem since I was about 12 it went from 1 lesion to about 20 in 5 years and I don't understand why? I have been having numb lips, eye pressure mainly behind the left but sometimes both, loss of balance, confusion, easily forgetting, mood change and I've notice I've lost a lot of energy I'm always tired and feeling weak. my middle ring and pointer finger on my left hand often go numb. I also notice sharp throbbing pains mainly in the right top side of my head but often varies and I get back pains and a stiff neck

Thank you for that additional info. I wish you could have provided the MRI report as well, there are clues in the distribution of the lesions and contrast enhancement which could orient the diagnosis.

The fact that your mother and her sister have similar issues makes the hypothesis of Cadasil really valid. There are familial cases of MS but most times it is sporadic, so I believe Cadasil should be considered. For that genetic testing is available to look for mutation in what is called the NOTCH3 gene, its finding would make the diagnosis. If positive your mother and sister should be tested as well as their MS diagnosis could be wrong. Symptoms apart from migraines can be very similar in both conditions as in both symptoms are due to white matter involvement. Spinal tap to check for oligoclonal bands which are present in MS is necessary as well.
