Suggest Treatment For Injury In Foot

Question: Hi. I twisted my foot about 7 weeks ago running. First my podiatrist thought it was a sprain since my xray showed nothing. He wrapped it with a medicated zinc oxide bandage for 2 weeks. After 2 weeks my foot was not better so I went and I saw another podiatrist. He gave me an aircast boot to wear for 4 weeks. After 6 weeks after my injury I still had pain in the middle of my foot while walking with the boot. So I went to an orthopedic surgepn who ordered MRI. The MRI report showed no fracture however the orthopedic surgeon himself saw the film and he noticed bruised bone. And I think he called it a contusion of one of the small bones (cuboid). Anyways up until now I have been wearing the boot for 6 weeks now. I can walk with the boot no pain however 2 nights ago I was walking barefoot and I tiptoed my foot and I heard a crack or popping sound on the other side of my foot- auch it hurt. Now it the crack position is not painful but looks like a pea size bone. Anyways how long will all this take to heal? I am tired of wearing the boot . How long will a take for a cuboid bone bruise to heal?????

The cuboid bone bruise usually heals in 6-8 weeks with proper rest and immobilization. The healing may be delayed if you continue bearing weight on the foot. Now that 7 weeks have already passed it is best to wait for another 1-2 weeks and avoid walking barefoot.
Do take daily calcium and Vitamin D3 supplements for an earlier healing.

I do take Vit D 2000iu every day. Does Calcium really help? My gynocolgist has told me before that I don't need it. I am confused about the weight bearing. It is ok to walk with the aircast boot, right? My doctor did not give me crutches. And after 8 weeks, how would I know that it is completely healed and can I wear regular shoes? When can I go back to my high heals?:)

Thanks for writing again.
Calcium supplements do help in early bone healing. You can walk with an aircast boot but never without it. You can take it off while resting on the bed. The proper healing can be detected radiologically. If there is no pain on wearing the regular shoes and weight bearing then also you can assume that complete healing has taken place.
Avoid high heels for at least another 2 months so that your bones and muscles do regain the proper strength.

How much calcium should I take daily?
I used to do Pilates. When do you think I can do pilates again?
In addition, the aircast boot has put pressure on my toenail (I have toenail fungus on my big toe) and has caused redness and swelling and pain around my toenail. Looks like ingroin nail. It is very painful. How do I treat it?

Hello.
You will need at least 500mg of elemental calcium daily with milk at bedtime.
You need to wait for at least 2 months more before starting with pilates, so the proper bone strengthening takes place by then.
Firstly, you may avoid using the shoes for a while and take rest till the lesion heals. If you are unable to get to a Doctor right away, you may follow these few tips.
Keep the area dry; You may use an antibiotic cream for local application on it. Apply thin layer over the area. It needs oral antibiotics and antifungal agents for proper healing that you can get from your doctor.

Try to keep the foot elevated over a few soft pillows for comfort and avoid contact of the area with the rough clothing. Avoid flexing the feet or toes as much as possible as that may increase discomfort. You may apply cool compress over the dressed nail if desired for comfort.

I routinely suggest my patients to take Cephalexin twice a day at least. In case the infection is too much with a pus like discharge then thrice a day dosage may be needed. I would suggest that you take it under the guidance of your doctor after seeing him on Monday
