Question: I sure hope so.
Every year, usually in Nov.~but this year not until XXXXXXX the toes on my left foot become inflamed, swollen and very tender to the touch. This happens to my 1st 4 toes, not the pinky. It usually starts when I've had shoes and socks on for about 30 minutes. The more I walk the warmer my foot becomes and the worse my toes become. Once I take off my socks the burning itching stops, but the toes remain red and swollen for quite sometime. And they are very tender. If I just brush them up against a blanket it feels like someone just sliced them open with a knife. It hurts so bad. So doc, what do I have???

I have uploaded 2 pictures. The first is what my toes look like right after taking off my socks. The second is a few hours later. I don't believe chillblains is what I have. That is cause as you said by extreme cold. My toes react to extreme heat inside my shoes/boots.

Thanks for the follow up and photographs.
You have developed raunauds phenomenon.
It is a cold related condition where vessels get constricted which makes the extremities very cold and white. Upon rewarming, the redness and sometimes pain start. I would advise you to keep limb warm and preferably avoid very tight garments and socks.

If there is no response tablet pentoxyfylline gives good response in such cases. I use them to treat my patients with similar problems and have seen very good response. You can try this after discussing with your doctor.

I understand what you are saying, but you start off with extreme cold issues. I stay barefooted when at home. My toes remain red, but are not swollen. They still hurt if I bump them even slightly. When I go out to do errands of course I put on shoes and socks. After walking in stores for even as little as 30 minutes my toes start to swell and get very warm to the touch. This is when they become very sensitive to touch, even my sock rubbing against them hurts. If I am unable to take off my socks the issue becomes worse and worse. Meaning the toes getter hotter, start to itch and I find it hard to walk correctly because the toes are rubbing on the sock.
Again, this only happen in the fall and winter months of the year. I live in NW Indiana.
I appreciate your attentiveness to my problem.

I will try to explain.
When you get exposed to cold, blood vessels get constricted and fingers become pale. This is subjective and in your case skin is still remaining red.
On exposure to heat vessels dilate and normal circulation begins, in your case vessels are dilating too much leading to sudden swelling and pain.
This is raynauds phenomenon.

That was a good detailed explanation Dr. However, my point remains that I am NOT exposed to cold prior to the exposure to heat while wearing socks and shoes. I will discuss this diagnosis with my family Dr.
