Brief Answer:
Looks like viral illness with rash

Detailed Answer:
Hi...Thank you for your direct query to me on Health Care magic.

Fever of 2 days without any localizing signs could as well a viral illness. Usually rather than fever, what is more important is the activity of the child, in between 2 fever episodes on the same day. If the kid is active and playing around when there is no fever, it is probably viral illness and it doesn't require antibiotics at all. Once viral fever comes it will there for 4-7 days. So do not worry about duration if the kid is active.

Rash developing on 2nd day that too on abdomen would most probably be viral infection induced one and I suggest you upload images of the same so that I can guide you even better after seeing the rash.

For fever - I suggest you use only Paracetamol (Crocin) in the dose of 15mg/kg/dose (maximum ceiling dose of 500mg) every 4-6th hourly that too only if fever is more than 100F. I suggest not using combination medicines for fever, especially with Paracetamol.

Meftal-P contains Mefenamic acid which is a NSAID. It carries the risk of gastritis and renal problems in the long run. Please do not use Meftal-P. I have published a scientific paper in a reputed paediatric journal that Mefenamic acid should not be used.

I do not approve of giving antibiotics like Augmentin (Amoxycillin-Clavulenate) too with proper investigations and especially without blood and urine cultures.

Please do not think that I am negating somebody's decisions, but I am advising this in the best interest of your daughter.

Please STOP AUGEMENTIN AND GET BACK TO ME WITH IMAGES OF THE RASH OF ABDOMEN.

Awaiting you reply - Regards - Dr. Sumanth

Brief Answer:
Looks like viral infection induced rash

Detailed Answer:
Hi...coming to your queries directly -

1. I have seen the photos - it seems like a viral exanthem, probably toxic urticaria or viral illness induced rash. This doesn't look like measles rash. Moreover measles rash starts behind ears and on face, not on tummy.

2. The other possibility is an antibiotic associated rash as Augmentin can cause this type of rash when used during viral illness.

3. The urine routine and urine culture reports - both are normal. This is not urine infection. Be rest assured.

I suggest -

1. Stop Augmentin and Meftal.
2. Continue paracip and Atarax.

I strongly feel it's a viral illness and the baby will be fine in another couple of days. Do not worry.

Regards - Dr. Sumanth
