Hello,
Thanks for the query.
According to the information supplied by you, your fasting blood sugar levels are little high and PP sugar levels are normal.
You can start taking glimmipride 1 mg one tablet before dinner to take care of your high fasting sugar levels.
Along with medications you have to keep observing dietary control and regular aerobic exercises for 40-45 minutes a day.
These life style modifications ( diet control and regular aerobic exercises )  have to be followed lifelong.
It is very important to keep a tight control on your blood sugar levels.
A meticulous control of diabetes will help in prevention of various types of future macro as well as micro vascular complications.
I hope it helps, however you may revert to me for any further query.
Best of luck
Thanks

Hello,
It is very unlikely that 1 mg of glimmipride would result in hypoglycemia. It looks unlikely that just 1 mg of glimmipride would result in crashing of sugar level from 154 to below 70 in the morning, resulting in hypoglycemia.
As far as microalbuminaria is concerned, you must get 24 hours urine albumin estimation done.
A level of albumin between 30 and 300 is called as microalbuminaria and is potential reversible stage. If the albumin level crosses 300 mg/ 24 hours urine, then it is called as diabetic nephropathy ,which is a dangerous situation because it generally leads to end stage renal failure in next 5-6 years.
Consult a nephrologist and get the kidneys evaluated properly.
Thanks

What is the diet management for above mentioned patient.....diet chart?
doctor
Answered by Dr. Pavan Kumar Gupta (7 hours later)
Hello,
As far as his diet is concerned,it has to be typical diabetic diet.
Since diet chart depends upon various factors like likes and dislikes of a person, preferences of veg or non veg foods, his working schedule etc,it is advised to consult a dietician in this regard.
However the much more important issue at the moment is evaluation of kidneys since patient is passing microalbuminaria.Remember that passage of proteins in the urine of a diabetic is a very dangerous issue.
Management requires arresting the process of decline in kidney functions and preserving kidney tissues. It is achieved by the following...
Meticulous Control of blood sugar levels
Control of hypertension
Avoiding kidney damaging drugs like pain killers, antibiotics like amikacin, gentamicin etc.
Prompt control of urinary tract infections
If XXXXXXX nephropathy develops, then avoiding potassium ( citrus fruits and coconut water )and a low protein diet is advised.
If XXXXXXX nephropathy has developed then, injection insulin is to be taken in place of oral anti diabetic drugs.
Must be given ACE inhibitors or ARB'S even if blood pressure is normal.
Regular testing for microalbumin in urine and serum creatinine levels.
If nephropathy has developed, then patient should be jointly managed by a diabetologist and a nephrologist.
Thanks
