Question: what tests would my mom need to rule out a heart problem. she hasn't been feeling well at all and I'm worried. she's 46 and smokes but she's doesn't have any health issues. she's been really stressed out and her chest is heavy and she asks she just doesn't feel well lately. I'm worried.

As your mom is experiencing chest heaviness and has at least one obvious coronary risk factor (smoking) it is necessary to perform a cardiac check up for ruling our any potential cardiac ischemia scenario.

A careful physical examination coupled with resting ECG are the first step to follow.

Also, additional tests are necessary:

- chest X ray study,
- cardiac ultrasound,
- exercise cardiac stress test (if sufficient suspicions of cardiac ischemia are raised),
- cardiac enzymes (CK-MB, Troponin)

If resting ECG, cardiac ultrasound and stress test result normal, then any obvious structural and functional cardiac disorder is excluded.

For an optimal prevention of future coronary risk, it is necessary to properly screen and control the coronary risk factors:

- periodically monitor blood pressure values (to detect any hypertension appearance),
- stop smoking and avoid close smoking contact,
- check blood lipid profile,
- check fasting blood glucose level,
- follow a healthy diet (Mediterranean diet is highly recommended),
- modify the daily life-style (perform at least 1 hour daily intensive physical activity, for example fast walking).
