Suggest Treatment For Heartburn And And Discomfort Instomach

Question: Hello there, I am a regular sufferer of heart burn (endoscopy done Feb this year found nothing significant) . Usually my heartburn symptoms are not relieved by OTC medications including ranitidine, calcium carbonate tabs etc.
Whenever I get severe heartburn I usually have to drink water and induce vomiting to clear my stomach to relieve myself of the heartburn.
Yesterday evening I did eat some fried stuff and started experiencing heartburn like symptoms after lying down. Tried a few antacids but did not work . I then induced vomiting as usual. This gave me relief for about half an hour or so.. Asmi tried sleeping I started experiencing moderate heartburn again. After another 2 hours of discomfort I drank water and induced vomiting again. Again after about 30 minutes the pain returned. If I keep walking about the pain is mild or gone. However if I try lying down the pain returns. Its been 4 hours since the pain started and I'm not able to sleep and have to keep pacing about the room to prevent the pain.

I am now starting to fear if this is heart burn or symptoms of a heart attack.

FYI my heartburn pain usually also causes pain in my jaw and back.. It happens every time I have severe heartburn and today was no exception.

My first vomit threw up lots of undigested food and wa very acidic. My second vomit was just acidic water.

I have gone through your query and would try to help you in the best possible way. I am sorry that you are suffering from such a great problem. From your statement i concluded that you are having heart burn problem and want to rule out if you have a heart problem. Endoscopy ruled out stomach problem. In my opinion there could be a number of causes and most common include:

1. Gastroesophageal reflux disease:
In this disease there is reflux movement of stomach contents in to esophagus resulting in retrosternal chest pain/burning sensation. It is more pronounced when lying down & after meal. It can be ruled out by undergoing lower esophageal manometry. It will tell if your lower esophageal sphincter has normal tone or not.

2. Heart Problem:
In case of heart problem sharp pain occurs in chest which may radiate to left jaw & arm. Along with pain there is profuse sweating. A family history of heart problem will bring more suspicion. As you had undergone ECG and which turned out to be normal so it seems to be unlikely. You can further clear it by undergoing cardiac enzymes analysis.

Suggestion:
As you have already undergone OTC medicines and they did not help so i would suggest you to visit a gastroenterologist for prescription medicines of proton pump inhibitors. Similarly cardiac enzymes analysis at hospital will also rule out cardiac abnormality.

General Care:
Following general care will help you in relieving the heart burn issue:
- Take frequent smaller meals.
- Don't take heavy meal at night
- Perform walk/exercise in the evening for at least 30-45 mins
- Eat fresh fruits & green leafy vegetables
- Yogurt & some pro-biotics

AVOID:
- Coffee or tea (both regular and decaffeinated)
- Carbonated beverages
- Alcohol
- Chocolates
- Smoking (if you do)
