Suggest Treatment For Hairfall Due To Dandruff

Question: i have been suffering from hair fall since long. but this time it has worsened. my hair are falling as of a patient undergoing chemo. i believe the possible causes of my hair fall could be dandruff and a constipated bowel system. pls suggest a quick fix for the dandruff to arrest hair fall the earliest. My hair have become too thin already. i tie them and i feel hair pull. for constipation, i am already taking triphala tablets daily. Pls help

Brief Answer: Antidandruff shampoo+steroid lotion for dandruff Detailed Answer: Hello and Welcome to healthcaremagic I am Dr, Kakkar. I have gone through your concern and I have understood it. Seborrheic dermatitis/dandruff presents as scaling and itching of the scalp. Increased hair shedding is also a common complaint associated with seborrheic dermatitis/dandruff If I was your treating doctor I would have advised you to use a 2% ketoconazole based shampoo, thrice weekly, for 2-4 weeks followed by one or twice weekly for maintenance. However if in case this proves insufficient OR sometimes from the beginning for a more complete and rapid effect, I usually ask my patients to use a topical steroid lotion (Mometasone is available as a lotion formulation) at the scalp skin twice daily for 2 weeks and once the dandruff is controlled I usually stop it and ask my patients to continue with just antidandruff shampoo for maintenance(twice weekly). Constipation has not been scientifically proven to be a cause of hair fall. Since you have been suffering from hair fall and thin hair since long, I would keep a possibility of either Telogen effluvium OR Androgenetic Alopecia, as the cause of your hair fall and decreased hair density. Scalp hair grows for 2-3 years (Anagen phase) after which it enter the catagen phase (transition phase) which is followed by resting phase (Telogen phase). Hair can prematurely enter from growth phase(Anagen) to resting phase(telogen) therefore resulting in shorter length and increased shedding. After a major stressing event like medical/surgical illness OR post-partum OR weight loss (in your case), scalp hair is prematurely pushed into resting phase (Telogen) from the growth phase (Anaphase). This phase usually starts in the third or fourth month after a major stressor and ends by six months if the stressor is removed. There are certain well known causes of Telogen effluvium: --Deficient Iron Stores --Hypothyroidism or hyperthyroidism --Post partum(after delivery) --Post Surgical or following a Medical illness (typhoid, malaria, dengue etc) --Weight loss/ Dieting etc Androgenetic alopecia can be difficult to distinguish from Telogen Effluvium in females, specially early in the course. Later on, Androgenetic alopecia presents as widened part-width and decreased hair density from the vertex and crown of the scalp. I would advice that you get a few lab test done e.g Complete Hemogram, Serum Ferritin levels, Thyroid function tests. In my practice I usually ask my patients of Chronic Telogen Effluvium to take an oral Iron Supplement along with a Biotin supplement for 4-6 months together. Follihair tablets are basically hair supplements and contain multiple ingredients like Biotin, Iron, Calcium pantothenate etc. They are helpful in controlling hair fall and help to restore good hair health. I think you can start with follihair tablets rather than taking triphala tablets However they need to be taken for at least 4-6 months at a stretch. The action is slow, therefore don't expect immediate results. It is advisable to take them for a good 1-2 months to judge whether they are going to be effective Or not. A good healthy diet, a gentle cleansing shampoo and a good conditioner can be used for dry frizzy hair. I hope this answers your query. regards

Thank you Dr. Kakkar. This helps. However i forgot to mention that taking any kind of iron supplements worsens my constipation problem. I will start with topical application of steroid lotion. Can you pls guide me on how to apply it, how much quantity to apply, when to apply? Will i need to wash my hair daily in the morning if i am applying this lotion? can you also pls suggest name of a good anti-dandruff shampoo which would be readily available in medical stores?

Brief Answer: Seborrheic dermatitis with hair fall Detailed Answer: Hi. Mometasone lotion can be used twice daily, in 1-1.5 ml amount, at the scalp skin, with a dropper. Elocon lotion is one popular brand marketed in India by MSD Pharmaceuticals. It has aqueous base and is therefore non-sticky or greasy and gets absorbed soon after application. No need to wash daily, you can wash on alternate days. However, if desired you can shampoo daily as well. Nizral 2% is marketed by Johnsons & Johnsons in India and is easily available. Regards
