Suggest Treatment For Fungal Skin Infection

Question: My son is currently in the hospital. I took him to the emergency room on 1- 13- 17. At that time he weighed 626 pounds, and was admitted with a fungal and viral skin infection, high liver enzymes, high CO2 levels and low blood Oxygen. He was doing better for two days, then his fever spiked and he was entobated . He was also put on dialysis which pulled off 4.5 leters each time. He was down to 508 pounds, wheened off vent and sent to physical therapy. on day three (3- 16- 17) He was barely responsive and CO2 levels were back up And was sent back to hospital where he is on Bi Pap 24 hours a day. IF he is off of it for more than three minutes at a time his blood Ox drops rapidly. Also his hemoglobin is low and has to be giving a pint of blood once a week. now down to 476 pounds, my question is what tests need to be given to find the causes. His doctors do not seem to be interested in finding out why.

I have gone carefully through your query and understand your concerns. Since there is response to administered oxygen and since the oxygen levels are dropping on its removal, it is clear that there is a problem with the gaseous exchange in the lungs and possibly interstitial lung disease is present. The oxygen therapy or bipap needs to be continued. Blood gas analysis may be done from time to time.

Apart from administering a pint of blood, perhaps intravenous iron therapy would be more valuable as that would help in resolving iron deficiency if that is the cause of the low hemoglobin level. I would insist you to upload the report of complete blood count. I need to know whether the anemia is microcytic or macrocytic as that will indicate the possible cause. Further tests can be planned thereafter.
