Suggest Treatment For Flatulence And Gas Pain

Question: My father had heart attack 7 months back and 2 stents were placed to LAD/LAX.He is fine after that and using medicines.he is getting heart burn when walking for a long distance say 3 KM and it goes of if he is resting. Today we consulted his cardiologist and he is suspecting another block and suggested us to undergo angiogram again. So can heart burn result to a block and lead to heart attack. Is angiogram required again. I am very much worried he is completely on a non fat diet but can blocks re appear even though he use the powerful cardiac medicines listed below. I doubt its a gas problem since he is suffering from gas problems and use to get stomach pain often. He is also not sleeping properly.Can you suggest me what to do.

1. A person with compatible profile, when complaints of heart burn or burning sensation in chest chest on walking or exercise which relieves on rest is highly suggestive of anginal pain. A person with such a complaint should be investigated further.
2. Yes, Despite all the cardiac medications and good diet control, the disease may come again. The block may appear at site of previous stent implant (restenosis) or it may occur at a new site. The time period of occurrence of RESTENOSIS is typically from 6-9 months are stent implantation.
3. I have seen the reports. They show satisfactory lipid profile, normal glucose levels and borderline creatinine level.

But the stents were medicated stents and why would a clot occur in medicated stents. Do you mean he really has a problem.Does any neurological problem also create blocks in the blood vessels.He feels light headedness and neck pain. He doesn't sleep well also.Are there any medicines for reducing the blocks. When should we go for stents like if the block size is more than 95%?Is the creatinine level any thing to worry

Dear Sir
1. The RESTENOSIS occurs due to fibrosis and not because of clots. It happens even in medicated stents although less frequently; 3-5% only depending upon the type of stent. Clot formation is a different process and it may also occur in medicated stents but that is usually an emergency.
2. Yes. The history is suggestive of angina. It should be evaluated further.
3. Neurological problems do not directly create blocks.
4. The medicines normally do not reduce block though a group of drugs called STATINs have shown to have reduction in plaques in some of the studies.
5. Theoretically a stent can be implanted when obstruction or stenosis is more than 70%. However, it depends upon many factor and the treating cardiologist is the best person to take such a decision.
6. The creatinine level is just above the normal limit. Not alarming but we should be careful about further damage to kidneys. Like we must not use any drug which can harm kidney. We must maintain good hydration and must use essential drugs (e.g. contrast materials) in minimum quantity for performing various tests and procedures.

I have seen angiography report, the attached four pictures of angiogram and recent blood reports. The angiography report says that there was obstruction in two vessels which has been tackled by stenting. The recent blood reports show low iron levels for which you can consult a physician.

I have seen the PTCA videos which were done at STAR hospital XXXXXXX on 17 march 2016. The videos are number till 52. They show that there was significant disease in two vessels and angioplasty and stenting to both has been done.

Dear Sir
1. There are a few minor plaques. They are not causing any significant obstruction. They are below the definition of calling them "BLOCKS" i.e 50% or more stenosis.
2. Medicines do not cause blocks to increase. They may decrease their growth. However, the disease process itself can cause the plaque to grow despite all the medication. Moreover, formation of healing scar over a stent may grow excessively and may cause restenosis as detailed above.
3. A small plaque may cause sudden total or near total obstruction if it ruptures. This process can not be predicted reliably. However, this mechanism is responsible mostly for sudden heart attacks.
