Question: I am a patient with fibromyalgia, diagnosed by my pcp and rhumatologist 17 years ago. I also have severe spine problems including degenerative disc disease, spondylitis?sp., bulging discs, one completely destroyed disc, with neuropathy in myleft leg. I had my left knee replaced in 2013 then had 8 really bad falls causing me to have revision surgery on knee. Same knee is popping out or off the " plate" twisting sideways and causing such severe pain it has caused me to pass out 3 times. My question is this, is there a Dr who would be willing to write a pain nevada strong enough to actually help, maybe even extended release once they review my records? At present, tne only thing my ortho used that worked was Dilaudid 8 mg 2 tabs three times daily with approval to use 1-2 in the miiddle of sleep cycle if needed. I am fully aware how large a dose that is. I am sure there is an equivalent in an extended formula similar to that. I currently have zero quality of life. No, I am NOT suicidal. Yes I am on Savella 100 mg daily, Lyrica 225mg twice daily, Trazodone 150 mg HS, Zanaflex 4 mg TID. I need relief and a sense of life back.

Hello dear.
I do a lot of pain management in my clinic and i think you are having the same problem of running over with many drugs.
In my opinion you can try with a transdermal patch formulation of buprenorphine 20 microgram.
This has to be applied once in a week on skin and it works for full 7 days. However, as it is an opioid based drug, you require discussing it with your doctor.
Also it may cause some constipation, so i will advise oyu to drink lots of fluids, eat diet rich in fibres like leafy vegetables and salads.
More over , i think your primary problem of fibromyalgia needs to be addressed vigorously.
You can also have another patch formulation if buprenorphine is not available. It is a transdermal patch of fentanyl which comes by 25 or 50 microgram and works for 3 days. Again please consult your local doctor for prescription.

However if you are looking for a doctor on this forum who can write and send you a script for pain medication, then I don't think there are any who would do it. Medical ethics doesn't allow us to do that.

No, I completely understand about the ethics. By trade I am a certified pharmacy technician but have been unable to work for the last 6 years. I am not drug shopping but I am looking for a good pain management doctor near XXXXXXX TN I also don't know how to talk to my DR without him/her thinking I am simply looking for drugs. I have been in physical therapy for 3 years for my various problems. I simply want someone who can do the facet injections and help me create a pain management plan so I can have my life back. I am also being treated for bi-polar depression with all this. So I am willing to try other options for my pain but I have already gone through so many I don't know what's left?

Hello dear. I can understand the problem you are facing. But only with a calm and quiet approach we can plan an approach for your pain management. See you have already tried various combination formulations. So i think in best judgment patch formulation for prolong periods will work for you.
Also it will help to cut down gradually your oral medications till the complete effect of patch arrives which is usually in 48 to 72 hours.
Again, also i would suggest you to resort to some alternative management like yoga, accupressure, behavioural therapies as these also help a lot to our patients.
pain management will require a multi modality approach in conjunction with your physician as well as your psychiatrist.
