Suggest Treatment For Fever, Sinus Infection, Abdominal Pain And Tiredness?

Question: My son is 22 and had a lot of problems lately without a diagnosis. It started in XXXXXXX with 103 fever out of nowhere and no other symptoms. Doc said sinus infection. Fever was gone in 2 days. After a few days, very tired, no energy, abdominal pain, could not tolerate dairy. Fever of 103 again in August, lasted one day. Still very fatigued, no energy, lost 15 lbs over last 4 months. Went to another doc, blood work showed elevated AST of 74, low vitamin D of 20.8, high platelet of 418. Normal EKG. All symptoms are now gone with the exception of daily abdominal pain after eating and feels full quickly. Pain is burning and dull located between sternum and navel, and will sometimes wake him up from sleep, never radiates. He thinks it's an ulcer and Pepto does help very quickly to relieve the pain. I'm wondering could it be mono or Lyme's disease. Should he have those tests run or does it sound like something else. The fever concerned me along with the high AST. Those results were from 3 weeks ago.

Also of note, he does not drink alcohol regularly but did drink a wine cooler the night before each episode of fever. He does not take any illicit drugs, and is now taking 2000 IU of vitamin D3 daily plus an iron pill daily. No other medications. He was tested for HIV, hepatitis C, all negative. No STDs as he is not sexually active.

It is dificult to say whether it is due to Lyme or mono. Because the symptoms you have mentioned are quite non specific and do not point towards any specific disease. Unless specific diagnostic tests are done it is difficult to say whether it is Lyme or Mono.

As far as the abdominal pain is concerned, it looks like duodenal ulcer only. An upper GI endoscopy may be done to confirm it. Tests for H.pylori infection may be done if it is an ulcer. Painkiller medications should be avoided. He can take Nexium for relief.

so if it an ulcer do they heal on their own or do you need a specific medicine and if it is from H. pylori, do you need to take antibiotics to get rid of that part? thank you so much for your help

Hi,
No, ulcer does not heal on its own.
He has to take antiulcer medications like Nexium for 4-8 weeks.
If he is positive for H.pylori he needs to take antibiotics for two weeks to eradicate it.
