Brief Answer:
one time failure does not mean problem

Detailed Answer:
Hello XXXXXXX

Thanks for writing to us. I can understand your concern.

One time failure n sex does not mean major issue.According to DSM 5 definition, Erectile dysfunction (ED) can be diagnosed when person fails most of the time (say more than 50) in last 6 months during intercourse. You are able to masturbate that means there is no major issue.

To help you further I need more information.

- Are you married?
If yes duration of marriage?
If not married, provide details about sex life with girlfriend. Where and in which circumstances you do sex? Also describe last night experience with new girl.

- How did problem start? Any specific incidence was there?

- Do you pay more attention to erection while trying for intercourse?

- Do you have any physical illness? Are you taking any medicines?

- How is relationship with your partner? Any emotional or social problem is there?

- Is morning erection present?

- Is there any history of smoking or alcohol?

- Is there any difficulty in urinating?

- When did you do successful intercourse last?

I will be happy to help further after your response.
Regards,
Dr.Chintan Solanki.

Brief Answer:
multiple causes are there, gradually accept the reality

Detailed Answer:
Hi,

Thanks for excellent information.

You have history of cardiac problem as well as gout. Almost all of the men who have cardiac disease suffer from erection problem. At age of 61 it is really difficult to treat. You are also not used to with sex that is another aspect. Performance anxiety of course plays a big role in such case. You need not be embarrassed by the comment of girl. I think considering your age you are ok. To see porn and enjoy masturbation is fine. If I were your treating doctor I would recommend following.

5- PDE inhibitor group of medicines like sildenafil or tadalafil are used for ED. One of them should be used hours prior to sex. They cause headache, stuffy nose nausea and gastric discomfort like common side effects. Tadalafil is preferred in patients with heart problem. Proper physical check-up of cardiac system should be done by a doctor before starting the same. These are prescription medicines and you need to consult doctor/psychiatrist/sexologist for the prescription.

One supplement mecobalamin and l-methylfolate (trade name tetrafol plus or metanex )is very helpful in my patients. It has shown improvement in 50-60 patients of mine with cardiac problem. You should take it after meal twice a day. If you feel some improvement in 20 days you should continue for at least 2 months. You may get this medicine over the counter as it is vitamin product.

Whenever you get chance of enjoying sex do it without any thought-apprehension of erection. Try to enjoy and you would succeed.

Take care.
