Question: I medicine for hypertension, sugar, cholesterol, am a heart bypass patient, have enlarged testicle. Viagra and other pills do not work. I have problems with erections. Have desire and feelings, am able to reach orgasm over time. What can I do to achieve and maintain an erection. I am 63 years old..

There are three options which can b useful.

Tadalafil if not used. This has shown efficacy in many of the patients who do not respond to sildenafil (viagra).

High dose of Sildenafil and other similar drugs. Silldenafil can be used upto 100 mg per day and can be taken safely. If you have tried lower doses only this is the option.

In case both the above options you tried already go for vacuum constriction device. This is useful method of gaining erection in those who erection problem following diabetes and hypertension.

Does the enlarged testicle have anything to do with the amount of blood going into the penis.? Why would the vacuum pump work over the drugs. Have you had good success with the pump..

Enlarged testis is not related with erectile dysfunction as both are distinct things.

Regarding vacuum pump it works because it is external device and stops leakage of blood from penis which maintains erection. Unlike pills it does not depend on the internal status and can work in dysfunctional state.

Yes I had at least 6 patients who responded to pump when drugs were not effective.

Vacuum constriction device is also known as penile or penis pump. They are available with prescription or without prescription. It is always better to get with prescription devices as there is risk of penile injury with non prescription devices despite having quick release feature in them. The device is somewhat different with prescription and needs detail demonstration by expert.

The constriction device can be left over penis after removal of pump for almost half an hour to complete intercourse.

There is few blood disorders which feature among contraindication to use penile pumps. On among them is sickle cell anemia and another is leukemia. There is need to rule out them before attempting use.
