Question: My boyfriend has had problems maintaining an erection when we have sex. He also fails to reach orgasm most of the time. This has been going on for about 6 months. He's in his mid-50s. When I asked him about it, he told me that he thinks my vagina is too loose. I can't bring him to orgasm with oral sex or masturbation, either, though. It's true that I've had three vaginal births, but I've never noticed a problem. I had three lovers before this boyfriend and everything felt and worked fine. For my part, I orgasm easily, have a strong sex drive, and no incontinence issues. He claims that he's never had this issue with previous partners either. What's going on?

Erectile dysfunction is very common in men and loose vagina is not the cause for him not able to maintain an erection or reaching an orgasm. Erectile dysfunction can be due to many causes so please get back to me with the following details

1. Does he have any co morbid conditions like hypertension, diabetes, etc?
2. Does he smoke? If yes how much and since how many years?
3. Please mention all the medications he might be on ( if any)
4. How long is the foreplay before the intercourse?
5. How many times a week do you both try to have sex
6. When was his last routine blood check up done?

Not that we know about. He is overweight and has become winded during sex. He's also on anti-depressants.

2. Does he smoke? If yes how much and since how many years?

No, but he did when he was in his 20s/30s.

3. Please mention all the medications he might be on ( if any)

I don't know the specific drugs, but he takes antidepressants and mood stabilizers every day. He has bipolar depression. He also takes an anti-viral (acycolovir).

4. How long is the foreplay before the intercourse?

It's usually pretty long, but he tends to focus on me.

5. How many times a week do you both try to have sex

It was maybe twice a week at first. But the frequency has really dwindled of late. And the last two times we tried at all, he said something wasn't right and we just stopped.

6. When was his last routine blood check up done?

Forever. He doesn't have insurance and rarely sees a doctor.

This is how you should go about this issue

1. First get a routine blood test done. Few conditions like diabetes, hypertension and cholestrol related issues can cause erctile dysfunction. Get a CBC, blood sugars and lipid profile done
2. Some anti depressants can cause erectile dysfunction. Please mention this issue to your psychiatrist during the next visit
3. Obesity itself causes erectile dysfunction. Strict weight control is of prime importance. Meet a dietician and also exercise daily for atleast 30 min
4. Taking cold showers and wearing boxer type underwear can also help
5. Tab Viagra can be taken once he gets his routine blood test done. 50 mg taken 30 min before intercourse can give you good results. Please ask your doctor for a prescription
