Brief Answer:
His Right kidney and Upper ureter are enlarged.

Detailed Answer:
Hi
I saw the report.
Pain in the right lower side of the abdomen can be due to Appendicitis or Right side Ureteric stone commonly.
As per Ultrasound report, stone has passed from the kidney into the Ureter and got blocked. As the stone is trying to move down, your brother is experiencing severe pain.Because of this, the Upper portion of Ureter and Kidney on right side are enlarged.
Because of the bowel shadows, the site of the stone and it's size are not clearly made out.The management depends on the size and site of the stone.
In my Opinion, he should get CT Scan KUB (Kidney Ureter Bladder) for exact size and location. Please consult a Urologist and proceed accordingly.
Hope I answered your query. If you have any questions, I will clarify further.
Wish you good health.
Regards
