Question: treatments for enlarged prostate in an 87 year old male with heart failure due to valve stenosis. the heart valve ejection fraction per the echocardiogram of a week ago was at 20%

There are two options of the treatment of enlarged prostate: surgery and medical treatment. For the surgery your doctor should evaluate your cardiovascular risks. As you have heart failure and low ejection fraction (20%) your doctor may evaluate you as a high cardiovascular risk patient during the surgery and concider that medical treatment is safer for you. Tamsolusin which you take is a gold standard if the treatment of prostate enlargement. If you start the treatment recently, then you should wait for some time to have the maximum effect. If you take tamsolusin for a long time and have no benefit then you should ask your doctor to reevaluate your risks and complains to see if there are other options.
