Brief Answer:
Possible underdosage of antihypertensive drugs. But rule out Secondary HTN.

Detailed Answer:
Hi Sir.

I understand your concern.

Elevated Blood pressure when on antihypertensive therapy could possibly result from inadequate dosage of antihypertensive drugs.

Simultaneously its also very important to screen for secondary causes of hypertension(Sec Htn) in any case of new onset of hypertension as well as recent sudden elevated blood pressure in spite being on BP controlling drugs.

Its very important to differentiate the above 2 causes for effective further management.

For this you need to undergo a complete Renal and Cardiac evaluation through Renal and Coronary Angiography.

In case if these investigations turn out to be normal, then you need not worry much and thereafter simple increase in the dosage of currently prescribed drugs is sufficient.

Thank you.
