default
Posted on Tue, 28 Apr 2015
Question: I am 40 year old male, with allergic to dust and earlier used to take levocet tab when used to get itchy eyes or continous sneezing. I am not taking this tablet now as am not getting those symptoms.

Currently there is kind of dark itchy rash on right edge of the nose and its increasing towards left side of the nose also. Seems to itch inside tip of the nose as well.

There is skin darking below the lower lips and chin as well. This one is not itchy. Only the nose is itchy.

I am only applying nivea cream as of now on face.

Please advise. Attached 2 image for review ( Face1 and Face2 jpg)

Brief Answer:
Possibility of seborrheic dermatitis

Detailed Answer:
Hello. Thank you for writing to us

I am Dr. kakkar. I have gone through details in your query. Unfortunately I could not find the Images. Kindly try and upload again.

I suggest a likely possibility of seborrheic dermatitis.
This condition commonly presents as red, scaly, itchy patches on sides of nose. Seborrheic dermatitis may also affect chin and area below lower lip as red, scaly patches.

However, I would like to confirm after I have a look at an Image.

Brief Answer:
Seborrheic deermatitis; topical steroids for a few days.

Detailed Answer:
Hi.

Thank you. I have reviewed the Images.
This looks like seborrheic dermatitis, as suspected.

Facial seborrheic dermatitis classically involves glabella, eyebrows, sides of nose, chin with itchy red and scaly patches.

I suggest you to use a moderately potent topical steroid e.g triamcinolone acetonide 0.1% cream Or fluticasone propionate 0.05% cream, twice daily for a few days. This should make it fine.

Brief Answer:
Use topical steroids Off and On if this recurs Or switch to pimecrolimus

Detailed Answer:
Hi.

Seborrheic dermatitis is endogenous, therefore, it may recur after seemingly complete cure. It usually remains localized to the sites I mentioned.

You may use the cream I have suggested, off and on, whenever and if at all, this recurs. Caution--Steroids should not be used regularly on face.
Alternatively you may use a topical immunomodulator e.g pimecrolimus 1% cream for maintenance. This is safe for mild recurrences and is safe even for long term use unlike steroids.

There is no oral medications for this. However for severe Or widespread recurrences Oral antifungal e.g fluconazole may be started. Never for limited disease such as yours.

There are no specific dietary advice regarding prevention of recurrences.

Regards
