Question: Hi I have been feeling unwell for a while and my GP thought it might be inner ear issues. I have dizziness, especillay on movement, nausea sometimes, lack of concentration, fatigue and pinpoint pupils. My blood pressure over the past day has been very low, between 71/56. and 88/59. What could be wrong. Also my pupils are very tiny.

Detailed Answer:
Hello!

Thank you for asking on HCM!

I carefully passed through your question and would explain that your blood pressure values are really low.

Your symptoms (fatigue, dizziness, nausea, lack of concentration, pinpoint pupils) can be explained with low blood pressure, which leads to low blood flow to the brain.

Are you taking any anti-hypertensive drugs? They can lead to low blood pressure.

Other possible causes to consider would be: an electrolyte imbalance, anemia, dehydration, a kidney or liver dysfunction, etc..

For this reason, I would recommend performing some tests:

- a resting ECG
- a chest X ray study
- complete blood count for anemia
- blood electrolytes
- kidney and liver function tests
- a cardiac ultrasound to examine your heart function and structure.

In the meantime, I would recommend having some rest, increasing salt intake and take plenty of fluids. I would not recommend performing physical activity because it can increase the risk of falling. Coffee would help.

If you are taking any anti-hypertensive drugs, I would recommend stopping them during these days, until your blood pressure comes back to normality.

You should discuss with your doctor on the above issues.

Hope to have been helpful!

Kind regards,

Dr. Iliri
