Brief Answer:
Superovulation, GIFT

Detailed Answer:
Hi,
Thanks for the query.
I understand your concern.
The possible causes IVF failure are, inducing less number of ovum due to the fear of hyperstimulation syndrome, which may not be sufficient in some women.
Some women may have poor ovarian reserve, which leads to development of less number of ovum.
By inducing more number of ovum by superovulation or by transferring the gametes into the fallopian tubes (GIFT) , this problem can be overcome to some extent.
Please discuss these things with your doctor and follow her advice.
Hopefully you next IVF cycle may become successful.
Take care.

Hell Dr. Sree,

Thanks a ton for your prompt reply, to give you more details, my wife is 36years & as advise we shall discuss with the doctor on the process mentioned.

In continuation with your advice & guidance, the following were the medicines & injections which my wife took before the IVF:
From Day of the periods which was 23rd March to 26th March Humog HP 150ml & Gonalf 150 ml. From 27th March till 1st April the dosage changed for Humog HP 225ml & Gonalf 150ml & Asporelix 0.25mg.
On April 1st midnight at 12 we were asked to take Ovitrelle 250mg.
Our IVF was on the morning of 3rd April.
Besides the above injections my wife was also prescribed the below medicines:
Codea plus, till the periods, Nutricell , Obfill diskette & Alphadopa for BP.

When i did my samen analysys i was told that my motility was only 17% & morophology around 3%, which apparently improved on the day of the pick up.

Please advise if the above medications given to my wife had to be altered so that she could have more ovaries formed in the folicles, our doctor told that on the left there was only sist & right side she could get 2 eggs out of which 1 was bad & the other was moderate. Please advise inspite of fertilzation what should we do so that folicles are formed for an embryo next time or higher eggs retreived for chances.
Please revert if i need to transfer any more consultation fees besides the one i send, wuold be pleased to comply if required.

Thanks again for your supprt.
Warm Regards,
XXXX

Brief Answer:
Can repeat the treatment or GIFT

Detailed Answer:
Hi,
Thanks for the query.
I understand your concern.
According to the history given by you, your wife has taken required treatment for superovulation.
Humog HP, Gonalf and ovitrelle help in superovulation.
So, your wife can repeat this treatment in next cycle and possibly better ovum development can be seen and second IVF can become successful.
GIFT(Gamete intra fallopian tube transfer) can give better results sometimes as the embryo gets natural environment for development here.
So, you discuss these things with your doctor and follow her advice.
Take care.

Thank you Dr.Sree,

Your support & response has been a big solace for us since we are going through the lows of this failure.
Will GIFT work even if i have a low motility & morphology? or it is only for normal seamen. Am currently on medeication for the past 2 months & have seen some progress.
Would any medication help to have better quality of eggs & more in numbers so that we have a better chance.
Apologies for all the trouble am giving by sending you back & forth XXXXXXX i would be pleased to pay any consultation fees, please advise if required.

Thanks again,
Warm Regards,

Brief Answer:
Motility of sperms is important for GIFT

Detailed Answer:
Hi,
Thanks for the query.
I understand your concern.
The results of the GIFT will be good with normal seminal parameters.
In case of abnormal seminal parameters, motility of the sperms is the main deciding factor of success.
As you mentioned that your seminal parameters improved with treatment, if your seminal motility increases to around 30% and if you are having good sperm count, then GIFT may attempted.
Though GIFT is sometimes good in poor ovarian reserve, in case of associated male factor abnormality it should be considered as second option. And it is more invasive as it needs laparoscopy and expensive than IVF.
The medicines your wife using are sufficient for superovulation.
Excessive usage of drugs can further decrease ovarian reserve.
Take care.
