Question: Dear Doctor,
I have been having stomach problems during the last 3-4 weeks. Took O2( ofloflaxin) 5 days, later flagyl 400 for 3 days and since these did not give relief took doxycycline 100 for 4 days. Still there is pain on the left Lower side abdomen particularly after food and loose bowel. Also taking. Vizylac. Can you pl advise what to do now. I travel a bit and this problem started during travel (probably from some inflight food)
Regards

Brief Answer:
It is traveller's diarrhea.

Detailed Answer:
Hi,
Welcome to health care magic,

Stomach upset with loose bowels and lower abdominal pain during or after traveling is actually traveler's diarrhea.

It occurs due to ingestion of contaminated food or old (not fresh) food or with unhygienic hands.

As you had already taken ofloxacin with metronidazole, vizylac and did not get much relief.

You should go for some routine blood works and abdominal ultrasound to rule out chances of colitis.

Here, I would advise you to take appropriate antibiotics (combination of norfloxacin with tinidazole) prescribed by some nearby GP or some local physician.

Vizylac or sporolac-DS will be good choice but should be taken 3 or 4 times daily to have a maximum benefit.

Econorm sachets or tablets (Saccharomyces boulardii) will also be helpful to treat diarrhea or stomach upset by restoring gut flora.

If lower abdominal pain is still there, you can take over the counter Tab.Cyclopam (dicyclomine hydrochloride 20 mg with paracetamol 500mg) with some antacid like pantoprazole or nexium (esomeprazole).

During traveling you should take pepto-bismol liquid or tablet 2 or 3 times a day but only after your doctor's prescription to prevent stomach upset and loose bowels.

Avoid eating uncooked or improperly food during journey.

Foods that freshly cooked recommended, such foods are safer than those on a buffet that have been sitting out for hours.

Clean your hands with soap and water frequently and before having meal.

Hope this will help you.
