Suggest Treatment For Depression And Blurred Vision

Question: My sir of age 36 years has been suffering from health problem for 3 years where the reason for has not been detected till now. once in a month he is finding pain in right lower abdomen if he gets that it leads to back pain with muscles and spinal cord and finally feeling psychological depression, severe anger and blurred vision . During that a little fever also prevails.he has gastric trouble.This problem gets temporary relief by use of any normal antibiotic course,but no doctor diagnosed a permanent solution.He has a history of cervical spondilytis two years back, By physiotherapy it cured to some extent.He has a history of use of sleeping tablets out of depression in life problems 3 years back.Now no such usages. 6 months back took an abdomen ultrasound scan and everything normal.In normal times he finds some numbness in legs after sitting so much time and walks with difficult. he is married recently 10 days back.

Thanks for your query for your Sir.

To recapitulate:
Male/36 - not well 3 years - *once a month - pain in right lower abdomen - > back pain > depression > anger > blurred vision > fever - gastric - OK with antibiotic - cervical spondilitis - no sleeping medicines now - ultrasound of abdomen 6 months back normal - numbness in legs after sitting long and walks with difficulty - married 10 days back.
Investigations show- ASO titre 400 - high rest is fine.

I would advise him the following:
- Tests of blood: CBC, ESR, Gold Quantiferone test for TB,
- CT scan of the whole abdomen.
- MRI of the spine
Neurologists opinion and lumbar tap for spinal fluid for analysis
X-ray chest
Mantoux test.
Blood culture when he gets pain and fever.

All these tests alone can find the basic cause.

Dear doctor,

My sir had undergone all the medical tests as suggested by you i.e.
CBC,ESR,CT SCAN OF WHOLE ABDOMEN,MRI OF SPINE,XRAY CHEST,MANTOUX TEST.

Among the above test i could not upload XRAY of chest and MANTOUX test but as far as reports are concerned as per a local doctor opinion the clinical study of both the test is normal with out any complications.

Regarding other tests, here i am uploading the reports.As per my knowledge, there is a problem with MRI of spine, please kindly go through the reports and suggest for further step to be taken regarding this heal problem.

Please provide me the detailed description of the problem not a brief one i am requesting it because the patient is in a depressed condition due to this prolonged unsolved problem.

Brief Answer:
Treat UTI and Neurosurgeon's opinion for correlation.

Detailed Answer:
Thanks for your feedback and the reports.
Two reports show abnormality.
The more important is Urine: It shows pus cells in large quantities, 12 to 15 and hence the protein+ and Epithelial cells. (UTI)
I would advise to send the fresh morning sample of urine in the sterile tube for culture and sensitivity and get started on proper antibiotics and complete the courses till totally normal. I hope this will solve all his problems.
Since CT scan and Ultrasonography is normal, there may not be any problems like a stone or so.

The MRI shows: there is a disc problem at L4-5, End plate of the degeneration of the disc and central disc bulge causing compression over the thecal sac compression. For this the correlation of the MRI finding with the clinical findings in the lower limbs has to be established by a detailed examination by your Physician or still better by a Neurosurgeon and his advised and orders to be followed.

Please follow this advise and I wish He should be fine.

Please let me know the reports and the follow ups.
