Suggest Treatment For Delayed Periods

Question: Hi Doctor,

I didnt get my periods till now. please suggest what should my further course of action. My last period date was 16th XXXXXXX 2015.

Thanks for the query to hcm for an opinion,
Considering the fact that you have been a diagnosed case of bilateral cod, and if you are not on any hormonal medication at present the menses shall be delayed and irregular. As you have been on treatment for infertility with PCOS, you have undergone ovulation induction and i believe that has been a failure. You have not mentioned as to how many times you have undergone the same?
THE MOST IMPORTANT THING in a case of PCOS is the insulin resistance and the level of hyperandrogenemia. The levels of serum insulin and serum testosterone , DHEAS have to be brought well within the normal range for ovulation to naturally occur and increased rate of conception.
Insulin resistance can be decreased by reducing obesity and by taking insulinogenic drug like metformin which you are already on 1000 mg daily. If your weight is still high and your serum insulin levels are still high then you require an increase in dose of the same and increased effort to exercises to reduce weight.
What is your weight and height? Your BMI should be below 30, ideally.
Presently as you have missed your menses by more than 10 days now, first get a urine pregnancy test and rule out pregnancy and then you can opt for a withdrawal bleed with progesterones to allow your menses to return.
I sincerely feel that you should get treated for PCOS first and bring your androgen levels to normal, before trying for conception.
This can be achieved by being on low dose OC pill like TAB GINETTE for atleast 6 months, this is the minimum time required to bring the levels under control. Then you may try for ovulation induction again and your chances to conceive shall fairly increase.
The decision is yours here and one needs to be patient. However i would like you to send me your lab reports( latest) and details about the number of ovulation induction cycles you have undergone till date.
