Brief Answer:
The reason could be psycologic or performance anxiety.

Detailed Answer:
Hi XXXXXXX

Welcome to health care magic,

As per your facts that you have posted here,it appears that you have problem of getting hard erection while attempting to have sex and late ejaculation when you use viagra.

This is kind of problems are often due psychological or mind related and due to performance anxiety.

Below listed measures will help you to boost up your confidence and getting good erection and have enjoyable sex.

1) Do regular exercises for about 30 minutes followed by meditation for 1/2 an hour in the morning.

2) Take high protein diet rich in vegetables and fruits and Vitamin A,C,D,E.and Zinc

3)Take anti oxidants like Almonds 5-6 everyday..

4) Avoid alcohol and smoking..

In addition to above, you will also need to rule out certain medical causes,
like diabetes, hormonal problems, blood flow problems, vitamin deficiencies, etc.)

For this, I would advise you to have the following tests:

1) Basic blood tests - including haemoglobin, blood sugar, lipid profile (cholesterol), urea, creatinine, Vitamin B12 / folic acid levels, etc.
2) Testosterone levels - total and free testosterone
3) FSH / LH levels
4) Thyroid function tests
5) Ultrasound Doppler of the penis (to check for any blood flow problems)

These tests will give you a clear picture what your sexual problem is due to - any hormonal problems or any other systemic problems or local problems in the penis. If there is any medical diagnosis then further treatment can be planned.

Please consult qualified Urologist for clinical assessment and get above basic tests done to rule out the cause of your problem.

Yes,you can have a child when you and your wife's medical reports are normal.
You need to have sexual intercourse in a fertile period of your wife's menstrual cylce i.e. if your wife has 28 days regular cycle and if the cycle starts from 1st date of the month then the ovulation will be on approximately 14th date of that month. So, you must have sex between 11th to 18th date which would be the fertile most period .

One more thing, you need to ejaculate the semen deep inside vagina in order to have fertilization of ovum by sperm.

Hope you will understand the above explanation easily and will get the benefit.

Thanks and Regards,

Brief Answer:
It can be possible with intracervical insemination.

Detailed Answer:
Hi XXXXXXX

I totally understand your concern what you wanted to say and advise you accordingly.

In your case, intracervical insemination (ICI) would be the easiest and most effective way to conceive.

Also it can be done at home and might require the help(advisable in your case) of qualified medical practitioner.

ICI can be tried in the most fertile period of your wife's menstrual cycle as I already explained it in my previous answer by counting dates of the cycle means calendar method or one can also use ovulation drugs or ultrasound in order to watch closely, the release of ovum.

Here, semen can be collected either by
1) Masturbation,
2) Nocturnal emission of semen,
3) Aspiration of semen(sperm) by means of puncture of testicle or epididymis.

ICI involves injection of semen into the cervix by a needleless syringe.

When fresh semen is used, one should wait till the semen liquefies then syringe should be filled with semen. After that the tip of the syringe should be put at the entrance of cervix or deep inside vagina and the plunger is pushed forward to empty the whole semen into the vagina.

The syringe should be kept at that place for few minutes and the lady should be advised to lie in that postion for about half an hour to improve the success rate for fertilization.

You may consult qualified medical practitioner like infertility specialist or gynecologist for this procedure to be done.

I think I have explained it in much details so that you could understand it very well.

Thanks and Regards,

Brief Answer:
Home insemination kits are available.

Detailed Answer:
Hi XXXXXXX
Thanks for the query,

Home Artificial insemination kit is available in market for intravaginal or intracervical insemination which has following contents,

1) Sterile container for collection of semen,
2)10 ml sterile syringe as the length of the vagina is approximately 10 cm so there will be no issue with length while injecting semen through syringe deep inside vagina or at the entrance of cervix,
3) Ovulation tests kit like LH surge kit to detect exact day of ovulation
4) Pregnancy tests kit for confirming success rate of the procedure i.e.pregnancy,
5) Thermometer to take basal body temperature
6) Detailed instructions for ovulation and the procedure for insemination.

Here in your case the problem might arise with the collecting procedure of semen from penis,so it would be advisable for you to consult infertility specialist for the whole procedure.

Average semen quantity would be around 2 to 4 ml per ejaculation in one time which will be decreased with subsequent ejaculation on that day.You can inject that semen collected from the said procedure and one can repeat the procedure after 30 minutes lag as I explained earlier.

The procedure could be done that whole week means from 11th to 17th but if you could know the exact date of ovulation then you can minimise those days up to 3 days means the day before and after of the ovulation.

It would be better you take advise from infertility specialist or gynecologist for the whole procedure.

I have tried my best to explain it as simple to be understood for you.

The success rate is approximately 30 to 50% and can be increased if both partners have their fertility reports normal.

Regards,
