Suggest Treatment For Continuous Irritation In Throat And Cough

Question: Hi. I have had some chronic problems that I have had since early childhood.
I have been coughing at night for as long as I can remember. It's a super wet cough. I get indigestion at night - my Doctor told me it could be GERD, put me on meds, and that did help a little.
I get EXTREMELY bloated and gassy after I eat, and I frequently have heartburn and almost always feel nauseous after meals. I'm constipated for days, and then I'll have diarrhea for a while.
It's very uncomfortable and hard to sleep, even when I eat early. I have taken sleep meds for years because otherwise I don't really sleep.
I also find myself very irritable. I will be okay for a while, then suddenly feel very tired and depressed, and honestly, just plain mean - for no good reason.
I feel stuffed up often, a lot of post nasal drip. I'll occasionally wake up (like once a week), with a rash on my face, most of the time in the corner of my eyes. It's itchy and it will swell up.
All of these problems have been pretty much constant for years and years now.

I have carefully worked through your case, and can realize your health worries. Being your physician, I will take care all of your medical concerns.

You definitely have GERD and it leads to stomach acid to reflux into the esophagus and straight up to the vocal cords and oropharynx area. It causes continuous irritation of the throat and coughing is due to the upper respiratory tract irritation and inflammation.

Reasons may be:-
Lower esophageal sphincter doesn't close properly, acid leaves out.
ulcers in stomach and duodenum.
H. Pylori infection, causing stomach and duodenum ulcers.
Hiatus Hernia (stomach protrudes upwards, through orifice in the diaphragm). It causes stomach acid to come in esophagus.

You need proper and more potent treatment rather more aggressively for this chronic problem.


If GERD is the reason and I was your treating physician I would have prescribed you with:
Dexilant
Citrulose
Ebastine
Azithromycine
Codeine Linctus.


If you are taking any other medication too, let me know for any alternative to be used for, as there may be some serious pharmacological interaction between them.


You will definitely get better in no time but if after 15 days of treatment, you feel nothing is working anymore, we will go for more investigations that may include but not limited to: H. Pylori infection testing, Esophageal lower sphincter abnormalities testing with barium swallow radiography, Endoscopy or esophageal manometery, Ulcer in stomach or duodenum testing etc etc.


Precautions:-
As you can't make a separate food for yourself, take little from food made for others and add major bulk from Salads and fruits making your perfect meals.
Avoid taking spicy, hot or very cold stuff, Junk foods, Soda, coffee, tea, beverages, alcohol, cigarettes etc.
Take proper rest and keep good physical activity.
Raise a bit the head end of your bed (don't increase height of pillows; place something beneath the head board of the bed to raise at least up to 30 degrees.) It will help in GERD symptoms improvement.

I do think the problem is starting in my stomach and causing me to cough, because on the days that I, for whatever reason, don't really eat, or I eat dinner very early, I don't cough as much.
Would it be enough to change my diet? Would it possible to alleviate the issues with just a change in what I'm eating? I will do whatever I need to do, and if that will do it rather than taking medication, I will take the extra time and make the extra effort to eat foods that will help me.
Should I try this first?

Yes, it is best to start with a change of lifestyle.

I happily welcome you back and want to reply your genuine concerns. I am happy to know that you are serious to change the fate of your health with your efforts and will power.


What can be better than defeating and treating a disease with diet and modification of lifestyle. I agree to that and repeat my aforementioned words that you need a bit change. So please consider the guidelines as mentioned previously and you will feel the difference.

But as this is a chronic problem and has been there with full bloom so if not aggressive, at least conservative and symptomatic treatment which can be managed with Over the Counter medicines is really necessary. So I would preferably suggest you to start taking:
Omeprazole
Citrulose
Loratadine
If cough, some cough syrup.

It will help relieve the symptoms soon. Also you need Listerine mouthwash gargles, proper tooth brushing and dental floss. Take plenty of plain water and milk (at room temperature).
