Suggest Treatment For Colic In A Child

Question: My son will be 6 weeks old in next three days. He was born at full term, normal birth. His blood group is O+ while mine is B+. I had no complications during pregnancy and after.
The baby was also doing fine until 2 weeks. He used to sleep most of the time and we had to wake him us usually to feed.
But now most of the time he just keeps on crying. He will be quiet only when i am feeding him. We have also observed that he is very gassy these days.
And he keeps on crying until he passes gas and motions.
I am exclusively breastfeeding him.
He passes urine atleast around 12 times in 24 hours.
He passes motion too atleast aeound 4 times. Infact he generally feels better only after passing mot ions else he will keep on crying unless i start feeding him.
His birth weight was 2.68 kg and was around 3.3 kg 6 days back.
We took him to the doctor for his inconsolable crying and they diagonsed it to be Colic and prescribed with Colicaid drops 3 times a day for 3 days.Should the drops be given ti him only when he starts crying with pain or we should give him once in morning afternoon and evening ?
Also, is it fine to co tinue with these drops on a daily basis ?

As of now i am not really very sure whether the drops helped or not as i have been a little irregular in giving him those.
Also, he makes a lot of grunting , gulping noises while sleeping. And no matter whether he has gas or nkt , he is not sleeping more than 2 hours at a stretch be it day or night.
And after waking up, he just keeps on crying until i start feeding him.

Could it be something else than colic ?
what else we can do to soothe him as most of the times when he is awake , he just keeps on crying.

Hi...Thank you for consulting in Health Care magic.

This is called evening colic and is quite common in this age group. This happens when the baby sucks at the breast very fast and in eagerness to drink milk will gulp in air too. Unless the air comes out like burping or flatus this discomfort will be there and next time check if the baby is sucking too fast and gulping in air too. You will be more convinced. Usually I don't advice any medicines for this as they give only temporary relief.

The two best ways to relieve this distress is -
1. Do not put the baby in lying position after feeding till the baby burps out the swallowed air.
2. If still crying - put the baby in prone position and keep patting the back gently so that the baby passes off the flatus and gets relieved.

Thanks for your quick response doctor. I am a first time mother and really not sure what is wrong with my baby. We usually burp him after every feed except at times when he will fall asleep while feeding. Even after 30 mins also, he will not burp and we just lay him down then.
Why he just keeps on crying everytime ? He will not get soothed even if we bounce him, cuddle him. He stops crying only if i start feeding him.
I was not getting enough breast milk during initial weeks and we had to formula fed him for the entire first week. How do i know if he is getting enough milk now as i am exclusively breastfeeding him.
Do you recommend Top feed ?
And his cryings are not only limited to evening or night. Today he started crying at 8 am and got calm finally at around 12 noon after he passed motions with gas. He was very sleepy as well but was getting up after every 5 mins in pains.
Apologies for being so descriptive but i am really scared of his inconsolable cryings now.He does not even let us burp him or XXXXXXX his back as he keeps on moving his limbs restlessly.
Shall i try Gripe water ? If yes, then at what time & how much quantity ?

Would really appreciate your time and suggestions.

6. At what age were the formula feeds started?
7. What is the exact date of birth and current weight of the baby?

Please find my answers below:
1. He will atleast feed for 30 mins which sometimes last till 40-45 mins
2. Lot of times he falls asleep while feeding & will usually sleep for 2.5 - 3 hours if he is not in pain. And when he does not want to sleep, then he will be quiet for around 30- 45 mins post which he starts crying again and i have to feed him to sleep
3. Yes, i have observed that but not always
4. We formula fed him just in his first week of birth and after that only if he was constantly crying. But just once in a day and that too just 30 ml. Never more than 30 ml / 1 feed in a day. He passss urine atleast around 10 - 12 times in 24 hours
5. His birth weight was 2.68 Kgs. 5 days back it was 3.3 KGs
6. We rarely formula fed him starting from 2nd week.
7. He was born on 22nd May and was 3.3 KGs 5 days back

Suggestions on breast feeding and formula feeds

Detailed Answer:
Hi....

Usually babies lose 10% of bodyweight in the first 10 days of life and regain it by 10-22th day of life.

He was born on 22nd may, so he's 38 days old now.

You said he was 3.3 kg 5 days back. So on 33rd day of life he was 3.3 kg.

We need to deduct initial 10 days as I have explained above.

He has gained 26.95 Gmail per day, which is appropriate.

Usually any baby who is gaining 25-30gm per day in the initial 3 months of life is a healthy baby.

Coming to your queries one by one -

1. Do not use Colicaid.

2. Do not give formula feeds by bottle. Use spoon or paladey instead.

3. Continue breast feeding.

4. You may or may not use gripe water. It neither useful nor harmful.

5. As such I recommend you to give exclusive breast feeding for a week and get back to me with his weight after a week of exclusive breast feeding. Based on this I will suggest you whether he needs formula feeds or not.
