Question: my mother is suffering from common cold, cough and fever (BP 70/40) she is paralyzed,hypertensive, diabetic and epileptic she is bed ridden AND catheterized from last two years and creation is 1.84 she is currently taking clopidogril+asprin75mg + ranitidine BD + PHYNYTOIN 50mg +levitracetam250mg TID +atorvastatin 10mg which treatment would you suggest and drug interactions with present medicine if any

I am Dr Rakesh Sharma answering your query.
I can understand your concern.
Before suggesting any medication, I would have liked to know how much temperature she having?
What all cough and fever medication she has received ?
If her blood pressure is 70/40 she should be shifted to hospital/ER right away.

If it is long standing hypotension, then she should be given as much fluid as directed by her treating doctor, as her kidney function is not normal.

Any way, Paracetamol is comparatively safe drug to give, if fever is high, that is more than 100F.

If cough is bad, then chances of Lung involvement is common in elderly individuals, so a broad spectrum antibiotic like cefexime and azithromycin combination can be given.

A honey based cough syrup like honitus will help her.

If required you can consult her treating doctor about this recipe.
Steam inhalation will be very useful for cold and cough.

If fever is very high, sponge with water.

However, I urge you to take her to the ER without delay.

Dear Sir,
Thanks for the prompt response. The Dr. has started Azithromycin and she is responding well BP is normal now and no fever but wet cough is still high and crefts in left lung can she take amoxi-clav 625 BD after this and is it safe to give anticold medicine (drug interaction) whether honey raise blood sugar plz suggest

Yes moxiclav can be given.

Sorry for delayed reply.
Amoxiclav is a very good follow up drug after this.Yes it can be given.
Anti cold medications have sedation, so it should be given in consultation with treating doctor.
In my clinical practice I have seen less variation in blood sugar after use of honey based cough , than normal cough syrup.
