Brief Answer:
I need further details.

Detailed Answer:
Dear Friend.
Welcome to HCM. I am Dr Anshul Varshney. I understand your concern.
You are giving her Paracetamol and Acelofenac/Acetaminophen for pain. But you have not mentioned where exactly she has pain , since when and what reason has been there for such pain.
Paracetamol doesn't has good analgesic actions. It mostly helps in slight pain.
So provide me details about her pain and all pre exiting medical issues so that I can help you.
Stay Healthy.

Detailed Answer:
Dear Friend.
Pain in shoulder after a fall can be due to muscular injury of fracture.
In this scenario I would advise you to get her MRI Of Shoulder done.
As far as pain is concerned I advise following in such situations for symptomatic relief:
1. Pain killer containing a combination of Tramadol and Acetaminophen available in market as Tab Ultracet. This can be given twice a day. Make a note that it causes sedation but an effective pain killer and safe for liver and Kidney as compared to others.
2. Hot fomentation.
3. Local pain killer gels like Volini three times a day.
I hope this helps.
If you have any further query please ask me.
Stay Healthy.
