Suggest Treatment For Chronic Rhino-sinusitis With OSA

Question: I have chronic sinusitis and mucosal thickening in maxillary sinuses diagnosed with a MRI (a year ago) and a CT last month when admitted to ER with a subarachnoid hemorrhage. I underwent surgery at Stanford (coiling) and I have minimal neurological deficits from the stroke. I have had a whooshing sensation in my head for several years. It does not seem to be rhythmic or related to pulse. Do you think this could be related to the worsening chronic sinus condition? I also have moderate to severe sleep apnea and do not tolerate the CPAP well (I believe because of sinus condition). I am 59, female, hypertensive (mostly controlled) and weight 155 pounds. Any suggestions?

As per your clinical history & CT MRI report it appears that you are having Chronic Rhino-sinusitis with moderate to severe sleep apnea .

Yes , extensive mucosal thickening in sinuses has impacts on your sleep apnea . It will worsen the condition.
Whooshing sensation in your head most likely because of chronic Rhino-sinusitis.

If I were your treating doctor, I would advice you
steam inhalation ,
Saline nasal spray &
Combination steroid nasal spray with antihistamine twice daily respectively.

You will also need complete ENT & Head Neck Examination .
Try to reduce some weight.

I would also advice you investigation like
CT Scan paranasal sinuses coronal cut to see detail anatomy & extent of disease in each sinuses ,
Drug induced sleep endoscopy to see % of obstruction at each level &
Thyroid profile to check level of your thyroid hormones.
