Suggest Treatment For Chronic Post-nasal Drip And Cough

default
Posted on Thu, 13 Apr 2017
Question: I had aortic valve replacement, about 2 months ago. Shortly after discharge, I started to cough, with tons of mucus being expelled. I just put it off as a chronic post-nasal drip (allergic rhinitis), but it has not been alleviated with anti-histamines, cough medicine, or, nasal sprays. In fact it is far worse than my allergic reactions have ever been. When the cough starts, it is almost impossible to stop it, and, it is such a very deep and productive cough, that a need to go through boxes of tissues. It is also extremely exhausting, as it seems very deep, and, there is so much mucus to be coughed up. It has even prevented my from going out, as, I fear the cough is so unpredictable, that I couldn't be with other people.
My doctor, and I, have left no stone unturned. Many tests have been done, my lungs are clear, and, we can't find any cause, or solution to the problem. It is driving me mad, as, it so depletes my energy, that I don't have much left to do the exercises required after cardiac surgery.
My doctor stopped my taking one of the blood pressure medications (candesartan), but, there seems to be no effect. Also, we have also tried acid reflux pills, which have worked for some. Not for me!
Do you have any other suggestions?

I wonder if this is an after-effect of my surgery, and, maybe it will disappear on it's own. I really hope so!

Brief Answer:
Have you ever undergone PFT?

Detailed Answer:
Thanks for your question on Healthcare Magic.
I can understand your concern.
I have gone through the detailed history you have given.
Excessive coughing with mucus production after Valvular surgery can be seen in bronchitis.
Bronchitis is inflammation of wind pipes (bronchi).
And for this you need to get done PFT (Pulmonary Function Test).
PFT will not only diagnose bronchitis but it will also tell you about severity of the disease and treatment is based on severity only.
You may need inhaled bronchodilators and inhaled corticosteroid.
Please let me know
1. Have you ever undergone PFT?
2. Have you tried inhalers before?
Please reply me answers of above asked questions, so that I can guide you better. I will be happy to help you further. Wish you good health. Thanks.

Yes, I have undergone PFT, in fact I had it just before surgery.
I do not have bronchitis, or bronchiextis (spelling?),or, anything else that I have been tested for. I even had a recent repeat chest x-ray, and, all seems to be normal. Also, bronchial dilators, or, inhaled cortisteroids, have not proven effective.
It's so frustrating, as, the cough is so unpredictable, and if it occurs, I almost sound like I have whooping cough!
There has to be an answer somewhere, but, both my family doctor, and I are stymied. I see my cardiac surgeon next week, and, maybe she will have some answers.
Meanwhile, maybe you can come up with some helpful suggestions.
I thank you for your interest.

I have just read about the benefits0f N-A-C.
It sounds like it may be exactly what I need to try.

I'm concerned about potential side effects, or reactions, with current medication I am taking (Bisoprolol; Pradaxa, and methotrexate (for rheumatoid arthritis).

The articles I have just read, seem to suggest that it is safe enough, with no adverse reactions.
What is your opinion?
The suggestion seems to be to take 600 mg., either once or twice a day.
What do you suggest?

I'm very excited right now, because this might be the answer that I have been desperately looking for.
I can't thank you enough.

You should take 600mg NAC twice daily.

Detailed Answer:
Thanks for your follow-up question.
I can understand your concern.

Your weight is around 67 kg, you will need NAC 600mg twice a day.
And no need to worry for drug interactions.

It is available in two forms.
1. Simple tablet
2. Effervescent tablet.
Both are equally effective, so start whichever you get.
