Suggest Treatment For Chronic Headache

Question: I've suffered from headaches for about 40 yrs. (I'm 75) They seemed to be sinus related. For the first yrs I took otc sinus drugs which helped at first if I took many. Sometimes an antibiotic helps. I've had sinus surgeries, however Xrays/CAT scans have never shown any sinus difficulties. Fiorinal w/codiene has been a miracle drug, and I would take 2 just every couple of wks as needed. However, now my headaches are daily. I've now been prescribed gabepentin, taken 600mg daily for a couple of months. It doesn't seem to help. Any suggestions?

I am Dr. Prakash HM and I will be answering your query.

Based on your query, my opinion is as follows:

1. Sinus problem can be solved with decongestants and if infections is severe, antibiotics will help.
2. Gabapentin is here prescribed for possible neuropathic pain. However, since its not helping, it may be not neuropathic cause.
3. As its in front of the head and above the eyes, ophthalmic cause or frontal sinusitis needs to be thought of.
4. An evaluation by a ophthalmologist will be necessary to rule out any diseases related to the eye.
5. Along with it, stress reduction, regular sleeping hours, avoid eyes strains and steam inhalation twice a day should be helpful. Steam inhalation, for five minutes, should help clear the sinus, if any. Involvement in social activity, principally outdoor will be helpful.
Do get an ophthalmologist opinion. If none, try to follow the above. Look for any aggravating factors for headache, if any.
