default
Posted on Mon, 17 Oct 2016
Question: Hi have chronic fatigue, even though I get plenty of sleep. I am often cool when others are not. I ache all over frequently. This is going on now for two months. Should I see a physician?

Regarding my chronic fatigue question, 3 additional notes:
no fever, BP 130/82 , not always-- but today was pale
and didn't feel like getting off the couch. Tired even
when wake up.

Thank you,

Brief Answer:
The most common cause is a low thyroid.

Detailed Answer:
Hello,
I read carefully your concerns.
The symptoms you described are suggestive for low thyroid ( hypothyroidism).
When I see someone like you in my practice, I typically order the following blood tests in addition to a detailed physical examination:

CBC (Complete Blood Count, also known as Hemogram; includes Hemoglobin, WBC and Platelet counts)
Electrolytes (Sodium and Potassium in particular)
HbA1c (Glycosylated Hemoglobin = your last 3 months' glucose average).
Liver function tests (SGOT , SGPT, Albumin, Bilirubin, Alkaline Phosphatase)
Kidney function tests (BUN, Creatinine)
TSH (checks your thyroid)
Free T4 (this too checks your thyroid)
25 hydroxy Vitamin D levels (ideal range 40 to 60 ng/ml = 100 to 150 nmol/liter)
Calcium

None of these tests require any fasting and can be done at any time of the day.
