Suggest Treatment For Chest Discomfort And High Creatinine Level

Question: Hi Sir, This is regarding my father in law, from past two weeks he was feeling discomfort in chest and was not able to eat anything due to vomit sensation, so we brought him to Apollo hospital here in XXXXXXX and found that the creatinine levels are high and asked us that he needs dialysis to be done immediately as this problem is related to kidney, so we had admitted him in hospital and performed dialysis every alternate days until the creatinine and potassium levels are in control ( when admitted the creatinine level was 6.2 gms/dl). later next week biopsy was planned, now since biopsy is done and reports are ready I would like to have opinions on how serious is the issue and is there any way to cure on medication rather than regular dialysis or kidney transplant.

Thanks for query .Based on the facts and all the relevant reports of your father in law I would state that his kidney functions are deranged due to long standing uncontrolled diabetes.

He was hospitalized and treated for raised serum potassium and creatinine and was successfully dialysed and his renal biopsy has revealed Diabetic Nephropathy .He is discharge from the hospital with creatinine being 5.5 mg .

Continue medications advised on discharge and follow the instruction .This will help to keep the creatinine levels within acceptable range .

However in view of diabetic nephropathy being a progressive disease he may need frequent dialysis as and when serum creatinine levels and serum potassium go above critical levels .

As regards your query for need for renal transplant ,considering his age being 62 years I would consider it as a last option .

Hi Sir, Thank you for the detailed explanation. We were discharged and again readmitted because my father in law was complaining about pain in kidney area where biopsy was done, upon bringing him to hospital and ultrasound done it was said that there was some blood leaking within kidney, so he is again under medication and complete bed rest. His age is 52 years and how safe is it to perform dialysis continuously when ever creatinine and potassium levels are high, can it be done life long, as doctor here was saying that after two years it will be necessary to do the transplantation of kidney and cannot continue with dialysis.

Bleeding from the site of puncture for doing biopsy is not uncommon and usually gets resolved with conservative management.

Dialysis is the technical procedure to eliminate waste products that are normally excreted by kidney .These products like urea and creatinine are poisonous to body .

Doing Dialysis has its own hazards .So it can not be permanent replacement to maintain kidney function and as I have stated earlier Diabetic nephropathy is a progressive disease a stage comes even doing a regular dialysis does not help to maintain creatinine levels at reasonably accepted levels compelling to consider Renal transplant as a only viable option.

Since he is 52 years old you can think over for renal transplant as and when deemed necessary.
