Suggest Treatment For Cervical Spondylosis And Acid Reflux

Question: I am 36 old Male from XXXXXXX Living in Australia, I have a few health concerns that I will explain here first before asking my questions.


Please bare with me as it is a little detailed. 


1. Cervical Spondylosis  - Diagnosed 3 years ago, was in severe pain for around a month and ever since being doing my stretching and Isometric excercises and it is under control.

2. Low back pains - Few recent episodes of Lower back pain, which seem to go away on its own in a couple of days, started seeing physio and seems to be ok.

3. Elevated LFT (Liver function test results), ever since I had pain killers and Anti-Inflammatory for my Neck pains, the LFT raised and slowly returning the normal, only after 3 years they came back to normal the last I checked was 5 months ago and I am not sure if they are still the same. 

4. Fatty Liver and little Fat in Pancreas - I have been told that i have fat in liver and little fat in pancrease, but have not been told what to do about it. The last time i checked my blood sugar it was 4.7 mmol fasting and 5.1 mmol after having food which the doctors said is normal readings. 

5. Acid Reflux and excessive bloating - Again from the time of the cervical spodylosis and after taking pain killers for over a month, my acid reflux seems to be constant problem. I take pantaparazole 40mg when needed, I once took it for more than a month and started feel wierd taste in my tongue, but what seems to give relief is having dinner few hours before i sleep and avoiding certain foods (chocolate, certain fried food), bloating has increased and I seem to have become sensitive to certain foods that were okay few years ago like cabbage, certain lentils. The gas gives me a lot of trouble. 

6. Blood Pressure - My blood pressure on an average has been 135/85 and I have been asked to keep an eye on it (When i visited XXXXXXX last year, i was asked to have metoprolol 12.5 mg every day but it started making me dizzy and gave me lot of palpitations (although i am told that prolomet is supposed to help with those symptoms and not create them, so i stopped taking them). 


I do have some stress at work and now i am a bit stressed with my health as well 


My Question is around - do these symptoms seem interlinked? what should i be doing for my bloating and acid reflux in particular? As my gas and acid reflux has sent me to emergency department for me thinking it to be heart and only to be discharged saying it is not the heart.

I am a Gastroenterologist and would be addressing your queries.
Have gone through your narration in detail and i appreciate your concerns.
As per your query answers are-
1) Yes most of these things are interlinked to each other. For example To get relief from pain of spondylosis you were on anti inflammatory drugs.One of the most common side effect of these drugs is severe gastritis.They stimulate the parietal cells in stomach and acid secretion increases.The extra acid is refluxed back into esophagus causing symptoms of reflux disorder.
The mucosa of our stomach has natural ability to withstand the effect of acid however the mucosal lining in esophagus get inflammed due to acid reflux causing symptoms.

Our gut consists of millions of bacterias and yeast which comprises the inner enviroment of our intestines called as gut flora or microbiota of our gut.This microbiota is very sensitive to any change in enviroment of body.Long term use of medications and toxins released by them destroys this microbiota causing problems with digestion like excessive bloating and belching.

There is a balance between Good and Bad bacterias in our gut but as colonies of good bacterias are depleted the food is acted upon by bad bacterias causing incomplete digestion and fermentation.This releases a lots of gases from the incompletely fermentated food causing severe bloating and belching.

Anti inflammatory medicines are metabolised by liver.This increases stress on liver cell causing mild inflammation in cells.This inflammation results in leakage of the enzymes(liver enzymes) produced inside these cells into blood hence causing deranged LFT.

So as you can see most of these things are inter related to each other.So if you remove the root cause things should probably settle down by themselves.

Blood pressure 135/85 mm of hg is prehypertensive stage and no medication is indicated only life style changes should be followed initially for 3 months to revert it back to normal.
Secondly metoprolol should not be used as a first line antihypertensive medicine.Ace inhibitors or Calcium channel blockers are first line antihypertensives.

2) Start taking Nexium 40 mg twice daily for 21 days.Along with it ask your Gastroenterologist to add a prokinetic drug like Levosulpiride once daily to the regimen.Nexium will prevent formation of excessive acid and would provide symptomatic relief in bloating while prokinetics would prevent reflux of acid back into esophagus.They should be taken intially for 21 days.
An antacid like Mucaine gel should be taken along with it three times a day before food to provide relief in acid reflux and gases.

Along with medications drastic life style modification is suggested initially for few months.

Avoid anti inflammatory.Take Ultracet(Tramadol plus Acetaminophen) if required.
Drink One glass of Chilled milk daily.Its alkaline in nature and neutralises acid.
Avoid spicy and oily food.
No pizza,burger or Fast food for few months.
No smoking or alcohol.
No chocolates or aerated beverages.
Take a Probiotic supplement daily.It would help to repletes the intestinal flora again hence improving overall digestion.
Avoid Vegetables and fruits rich in fibers.
Don't take juices of citrus fruit as they cause gastritis.
Eat home cooked unprocessed light easily digestible food for few weeks,Take lots of curd daily.

Follow what i suggested for 21 days and revert me how you feel after that.Have a realistic approach things will take time to settle down.

thank you for your detailed answer doctor, my doctor has put me on nexium today just like your suggestion. can something like chana dhal make you feel quite sick, because it makes me sick for a few hours so I have stopped having it.

Chanadal is heavy and difficult to digest,Try taking Green Moong ki daal for few days dear.
Taking Chanadal can aggravate your problem so avoid it for few days.
Take light and easily digestible food.
In case you can get Bail murraba(Take it twice daily) you need to search it online in australia. It will help you a lot.
Take Triphala churna 1 tsf before going top bed and ashwaganda one capsule daily.

Follow up after 2 weeks.

hello doctor, thank you once again for a prompt and great reply. I have a follow up question. I can create a new question if you like or I can ask here.
I did blood tests yesterday, my doctor called me to say that amylase is 150 U/L and it should be 0-120. she said my last blood test showed it to be 120 and as it has increased better to do a CT SCAN. My lipase levels are at 22 U/L
I have booked my CT scan, but basically is this a worrying report?

Normally, only low levels of amylase are found in the blood . But if the pancreas or salivary glands become damaged or blocked, more amylase is usually released into the blood. Blood amylase and lipase levels are most frequently used to diagnose pancreatitis or any other pathology involving pancreas. When the pancreas is inflamed, increased blood levels of amylase and lipase, pancreatic enzymes, will result.However a minor increase in level of amylase with normal lipase has no significance so i don't feel you are at all at risk of pancreatitis or you needto worry about mildly elevated levels. Amylase and lipase levels should be increased up to 3 times normal for having any clinical significance. Secondly,both values should be increased, in order to carry the diagnosis of pancreatitis or any other pathology involving pancreas( Infact lipase is more sensitive then amylase for pancreatic pathologies). However as you are in a developed country right now going for a CT scan is always a good option. Because when we have investigations and technology then why to take even 0.01% risk.
