Suggest Treatment For Burning Sensation In The Throat When Diagnosed With GERD

Question: I have a severe burning sensation in throat, eyes tear and burn. Have had bouts of Gerd but could normally get under control. This latest episode started 0ct. 31. 2016. I have had this 24/7 since then. I feel it was brought about by iron infusions I had done. 1 in july 1 in sept 1 oct 25 and 1 oct31. Nov 1 the burping from my stomach started and the gas bubble comes up burns my throat. eyes and nose. It is horrible and has disabiled me. tried 5 different proton pump inhibitors, no success had endoscopy and nothing showed up. If it is the iron how do i get it out of my system?

No it has nothing to do with Parentral iron therapy.

Firstly i would say that parentral therapy with iron is not going to increase symptoms of GERD.So leave it,it has nothing to do with it.
Oral iron therapy can cause acidity,loose stools etc but parentral therapy has nothing to do with your symptoms right now.

From your history its apparent that you have tried only proton pump inhibitors till date,So please don't panic.Have faith in me. GERD and its symptoms can be controlled very well by a number of latest molecules and combinations available and have been tried successfully.

The problem you are suffering from is called Gastroesophageal reflux Disease. Gastroesophageal Reflux Disease is a form of digestive disorder, which affects the lower oesophageal sphincters or the muscle ring present between the stomach and the food pipe. GERD causes return of the contents of the stomach back to the food pipe. Heartburn, acid indigestion, is the primary symptom of GERD. The affected person may feel a burning chest pain which starts from behind the breastbone and reaches up to the throat. The feeling can be defined as food coming back to the throat with a bitter, acidic taste.All your symptoms are here in it.

Firstly i would suggest you to switch over to Nexium(Esomeprazole),Initially take it 40mg twicedaily,However if not relieved doses can be increased to 80mg twice a day. Esomeprazole is far superoir to other PPI's in treating reflux disorder.
Other then that ask your gastroenterologist to add Prokinetic drug like
Levosulpride;It is a sulpride isomer and exerts its prokinetioc effect by blocking D2 receptors,Recent studies have shown that combining it with Rabeprazole has shown good effect in patients suffering from GERD.

Domperidome-Very effective in treating GERD when combined with Rabiprazole.

Acotiamide-Trials showed that 30mg doses taken for 3 month thrice daily were highly effective in treatment of of functional dyspepsia.The positive effect of acotiamide on both gastric emptying and gastric accommodation,Gastric accommodation and gastric emptying in acotiamide group were significantly enhanced in people taking it.

-Baclofen- normally used for cerebral palsy patients and patients with other neuromuscular diseases.It has been successfully used in patients with reflux.It may decrease the frequency of relaxations of the lower esophageal sphincter and therefore decrease gastroesophageal reflux.

Test advice-
Endoscopy to examine the inner part of the oesophagus.
Ambulatory acid test for monitoring the amount of acid within the oesophagus.

Apart from that drastic life style modification and dietary changes are required-
Firstly stop taking soda or any aerated beverage.
Food and beverages like chocolates, fatty food, coffee, alcohol and peppermint should be avoided in order to reduce the weakening of the lower oesophageal sphincter. Several food items irritate the damaged oesophageal lining and should also be avoided. They include citrus fruits, tomato and pepper.
Smoking cigarettes results in weakening of the lower oesophageal sphincter and should be stopped to reduce GERD.
-Take about 30 grams of fibre daily in your diet.Eat one cup of fresh fruits(except banana) and half bowl of green salad daily;this will meet your extra total fibre requirement.Avoid cirus fruits as they can increase symptoms.
- Avoiding alcohol.
-Exercise daily for 20 minutes at least.
-Avoid oily and spicy food.
-Drink COLD milk.
-Yogurt is a very good natural source of probiotic take it daily.
-Try to stop taking prednisolone it increases gastritis.
Adhering to both medicines and Strict life style changes will help you in long term.
-Last but not the least ,Keep yourself fully motivated.You have just started the treatment miss,Proton pump inhibitors are first line drug;a number of drugs and combinations are still left.So don't panic Just discuss options i told you with your Gastroenterologist. Follow drugs and Follow restriction,You will feel much better in a week or two.

THANK YOU FOR YOUR RESPONSE.I NEVER EAT OFFENDING FOODS,SMOKE OR EVER DRINK ALCOHOL. I ALWAYS EAT VERY BLAND DIET, SO THATS WHAT IS SO BEWILDERING TO ME. THEY WANT TO DO A MANNETRY TEST, TO CHECK THE FUNCTION OF ESOPHOGAS AND XXXXXXX MUSCLE. I COULDN'T DO IT AWAKE, NOW THEY WANT TO SEDATE ME. I'M AFRAID THEY WILL SCREW SOMETHING UP PUTTING THAT TUBE UP MY NOSE AND DOWN INTO MY STOMACH. DO YOU THINK THAT WOULD BE OF ANY BENEFIT TO ME.

Good to hear that you are strictly following dietry restriction but along with it medications have to be taken.Then only things are going to change dramatically.
Manometry will help you.Esophageal pH monitoring is a procedure for measuring the reflux of acid from the stomach into the esophagus that occurs in gastroesophageal reflux disease.Dont worry its a safe test and even i had suggested you to do this in my answer.
Don't worry its a simple test.
Apart from that i want you to discuss two things with your treating doctor-
1)Add Levosulpride to Rabeprazole.
2)consider baclofen or acotiamide.
