Brief Answer:
Carcinoma left breast

Detailed Answer:
Dear XXXXXXX
I have gone through the details of your Mom.
Earlier she must be having a benign lump in the breast which has now transformed into malignant lesion. The lump is mainly outside and has spared the underneath structures. There are doubtful nodes present in the left armpit. The pet scan and the ultrasound scan show the same thing and rest of the body parts appear to be normal.

It is essential to undergo surgery first in which case there is removal of the affected breast followed by lymph gland dissection from the axilla. The dissected specimen is sent for histo-pathological examination and marker assays by which we can determine the further management after surgery.
Since there is skin infiltration she may need Radiotherapy as well followed by chemotherapy at least 6 to 8 cycles. This will be followed by oral hormonal therapy depending on the hormone assay of the tumor.
Over all your mom will have normal life ahead after complete treatment. Only surgery will not be a good gesture. There will be some side effects of the treatment but she will have good and normal life ahead.

Dear XXXXXXX
I replied your query only after seeing all the investigations including PET. As I told you that it is only a local disease and after treatment as advised she will have completely disease free life and chances of recurrence will be remote.

Brief Answer:
I agree with your doctor

Detailed Answer:
Thank you Mr. XXXXXXX for reverting.
Since your Doctor has physically examined the patient and must have found the surgery at this stage will not be fruitful. To make the surgery successful it must have become necessary for him to give chemotherapy first. It will shrink the tumor and make surgery easy. It will also save her from the spread of the disease. It is called the Neo adjuvant Chemotherapy. This is more appropriate method of treatment.Chemotherapy followed by surgery followed by radiotherapy and then hormone therapy if receptors are present, is the rule. As such your doctor is right in saying so and you should trust him. Only you have to ensure the experienced surgical Oncologist takes care of the surgery.

Dr. J. Ticku
