Question: History : Inflammatory arthritis 5 years, Lumbar disc herniations /degeneration. Meds: Oxycodone 30mg x 5 prn Prednisone 7mg ....currently tapering with great difficulty due to adrenal fatigue but will be switching to biologic. Problem: After high levels of stress both emotional and physical thurs / Friday I started having symptoms of moderate adrenal fatigue followed by severe LLQ cramps and nausea that produced ragged stool, (W/ ammonia smell) and some coagulated red blood and mucus. Saturday morning I had 3 similar movements with blood/ mucus but no cramps or ammonia odor. Sunday nothing just RUQ gas like pains, felt constipated and some nausea, felt weak with head rushes on standing yet BP normal. Today I seem constipated and have constant nausea, colon is slightly tender on palp., no rebound pain, bowel sounds seem normal (Ret. paramedic) Did have some chills Friday but no fever, pulse BP. normal. I should mention that I upped the prednisone to 15 mg for 2 days and resolved many symptoms. Took 2 doses of Miralax since yesterday with no movement. No history of GI other than opioid constipation that is well controlled w/ Miralax and high fiber diet ....normal movement every 2 days. Wonder what you think the crampy / bloody toilet episode may be AND if I should add something like Senna to the constipation. Ischemic colitis or cancer are the best matches I find. Appointment with Rheumatologist Wed. Oh BTW...52 yr old male, active aside from arthritis restrictions, good health otherwise

Brief Answer:
hi, various reasons for this

Detailed Answer:
Hi, the bloody stools and cramps may also be due to
1. Infectious colitis
2. Hemorrhoids
3. Fissure( given that you have history of long standing constipation).

You shall add laxatives to take care of constipation. Other symptoms may alsi get relieved with this.
Since youbare on steroids and tapering them now, chances of viral or bacterial colitis are all the more.
Rest of the symptoms are, as you know, due to adrenal insufficiency.

It is not so common for cancer to present with such short history. Therefore less likely.
